Global Small Bowel Motility: Assessment with Dynamic MR Imaging

Abstract: To assess the repeatability in human volunteers of software-quantified small bowel motility captured with magnetic resonance (MR) imaging and to test the ability to detect changes in motility induced by pharmacologic agents.

“…To place the mean motility (0.25 AU) of CIPO patients into context, a recent study of healthy volunteers reported a mean global motility of 0.16 AU following injection of the spasmolytic agent hyoscine butylbromide . It is thus clear that CIPO patients retain some intrinsic motility following oral mannitol and that bowel is not completely aperistalsis.…”

“…Following acquisition of the baseline motility scan, 11 controls and 7 CIPO patients (without contraindication to neostigmine) received either a 10 mL IV saline bolus or a 10 mL IV bolus of saline containing 0.5 mg Neostigmine Methylsulphate (Mercury Pharma, Croydon, UK). Nine of the 20 controls received IV Buscopan as part of another study . Neostigmine stimulates bowel motility and often relieves symptoms in CIPO.…”

“…Registration provides (i) parametric surrogate markers of motility and (ii) allows automatic propagation of regions of interest (ROIs) through time series data once an ROI has been defined on a single image . This type of analysis has produced objective and repeatable metrics of global small bowel motility . Until recently, the most widely used registration technique was only applicable to breath‐held data as breathing produces additional motion within the images that is not related to the GI motion of interest, thus confounding the data, by mimicking bowel movement.…”

“…The software quantifies color intensity changes in the bowel fluid, resulting in a surrogate measure of motility (Figure ) . The technique has been used in a study to evaluate changes in motility after administration of neostigmine and butylscopolamine . A significant global motility increase for neostigmine was found, compared to placebo, and a significant motility decrease for butylscopolamine.…”
