Giant Nondural-Based Cauda Equina Meningioma with Multiple Cysts

Abstract: A very rare case of a giant nondural-based cauda equina meningioma with multiple cysts was presented. Spinal meningioma most commonly occurs in the thoracic or cervical region and typically adheres to the dura. Only six cases of nondural-based meningioma have been reported in English literature. All occurred in the cauda equina region. These patients were predominantly female and younger than those with typical intraspinal meningioma.A 46-year-old woman had a 4-year history of lower back pain and right leg pai… Show more

“…However, lumbar spinal meningioma in the elderly patient is especially rare. In one series, lumbosacral meningiomas represented only 2.3% of cases among 705 spinal meningiomas [4]. In two other series meningiomas in patients 70 years or older represented a maximum of 10% of all spinal meningiomas [10,13].…”

“…The patient had no history of neurofibromatosis. The differential diagnosis for our patient included schwannoma, cystic neurofibroma 10 , myxopapillary ependymoma 22 , cystic meningioma 23 , and cystic teratoma 24 . Although there are no pathognomic radiographic findings for schwannoma, rim enhancement of the cystic potion of this intradural extramedullary tumor is highly suggestive of the diagnosis 25 .…”

“…Spinal meningiomas typically attach to the dura, and non-dural spinal meningiomas are rare 1 , 7 , 10 , 12) . Eighteen cases of non-dural based spinal meningiomas have been reported to date ( Table 1 ) 2 , 3 , 4 , 6 , 8 , 9 , 11 , 13 , 14 , 16 , 17 , 18 , 19 , 20 , 21 , 22) . Most tumors in these cases occur in the lumbo-sacral region (16 cases), including the thoraco-lumbar junction (two cases) 2 , 3 , 4 , 6 , 8 , 9 , 11 , 13 , 14 , 16 , 17 , 18 , 19 , 20 , 22) .…”

“…Most of histopathologic subtype of non-dural-based spinal meningiomas was clear cell meningioma (15 cases) 2 , 3 , 4 , 6 , 8 , 11 , 13 , 14 , 16 , 18 , 20 , 21 , 22) , whereas the remaining three cases were angiomatous, transitional, and meningotheliomatous meningiomas 9 , 17 , 19) . The clear cell meningiomas were quite predominant, as high as approximately 83.3%.…”
