Giant intravenous glomus tumor

Abstract: We report a painful intravenous glomus tumor located in the right forearm of a 79-year-old woman. The tumor originated from the wall of a vein, protruded into its lumen and was completely excised. The largest dimension of the tumor occluding the vein was 14 cm. Tumor cells were characterized immunohistochemically by the presence of vimentin, alpha-smooth-muscle actin, and collagen IV. Intravascular spread of the glomus tumor is rare and has been described in the stomach and subcutaneous tissue. An entirely int… Show more

“…The only other report was recently published and discussed a case of a 55-year-old woman with a 2.5-cm glomangiomyoma in her left kidney. 1,4,6,10,11,15,17,18,20,[28][29][30] Tumor immunoreactivity for desmin was variable, ranging from no expression, like our cases 4,11,15,18,20,29 to only focal positivity in other studies. It is stated that the tumor was encapsulated and distinct from the renal parenchyma.…”

“…Most of these cases present as subcutaneous masses of the distal extremities particularly in the subungual region. 1,18,20 In the kidney, an uncommon location for mesenchymal tumors, only 2 examples of glomus tumor have been reported. 10,12,31 In occasional cases, the tumor had an intravascular growth pattern.…”

“…In addition, mast cell of the glomus tumor, may play an important role in the mediation of pain [16]. As previously mentioned, the forearm is the most common extradigital location [3], and the lesions most commonly occur in the subcutaneous layer, with very infrequent occurrence in extracutaneous locations such as muscle, bone, and blood vessels [1, 68, 17]. Superficially located glomus tumors often produce typical symptoms such as paroxysmal pain, exacerbated by changes in temperature [9].…”

“…Although the forearm is the most common extradigital location [3], intravascular origination of a forearm glomus tumor is an exceptionally rare occurrence. To the best of our knowledge, only 3 cases of intravascular forearm glomus tumors have been reported [1, 6, 7]. Here, we present a rare case of a patient with chronic pain and focal tenderness caused by an intravascular glomus tumor in the forearm.…”
