BackgroundDetermining what constitutes “good practice” in the measurement of the costs and effects of health promotion and disease prevention measures is of particular importance. The aim of this paper was to gather expert knowledge on (economic) evaluations of health promotion and prevention measures for children and adolescents, especially on the practical importance, the determinants of project success, meaningful parameters for evaluations, and supporting factors, but also on problems in their implementation. This information is targeted at people responsible for the development of primary prevention or health promotion programs.MethodsPartially structured open interviews were conducted by two interviewers and transcribed, paraphrased, and summarized for further use. Eight experts took part in the interviews.ResultsThe interviewed experts saw evaluation as a useful tool to establish the effects of prevention programs, to inform program improvement and further development, and to provide arguments to decision making. The respondents’ thought that determinants of a program’s success were effectiveness with evidence of causality, cost benefit relation, target-group reach and sustainability. It was considered important that hard and soft factors were included in an evaluation; costs were mentioned only by one expert. According to the experts, obstacles to evaluation were lacking resources, additional labor requirements, and the evaluators’ unfamiliarity with a program’s contents. It was recommended to consider evaluation design before a program is launched, to co-operate with people involved in a program and to make use of existing structures.ConclusionWhile in in this study only a partial view of expert knowledge is represented, it could show important points to consider when developing evaluations of prevention programs. By considering these points, researchers could further advance towards a more comprehensive approach of evaluation targeting measures in children and adolescents.

It is a desirable medical as well as health economic objective to achieve the best possible health effects with given financial resources. Estimating the costs of intervention programs is complex and not always possible in advance. One possibility to obtain information on costs or cost effectiveness of certain interventions is to assess programs already in existence. The aim of this article was to develop a simple, easy to understand and practical possibility of documenting costs of prevention and health promotion programs. Based on the fundamentals necessary for cost assessment, a questionnaire and a module to document the consumed resources and the resultant costs were developed. These were applied and improved within a pre-test. The developed cost module is as follows: In the left column, 5 key cost categories are listed: personnel, rooms, equipment and process, management, other costs. The cost module is a compromise between different objectives that are difficult to reconcile. On the one hand, the costs should be documented as detailed as possible and on the other, the module must be very simple to implement, as otherwise it will not be used in practice. For this purpose, it might also be useful to develop a module for each stakeholder that is aligned as closely as possible to his or her special activities. All feedback and suggestions from the pre-test were incorporated. However, some of the feedback points were project specific. Here an attempt was made to find a compromise between detail and practicality. This was done by implementing more detailed descriptions and examples in the manual. The presented module is very general. This also is a great advantage because it can be used to document the costs of completely different stakeholders. Thus, the concept presented here for cost assessment provides a first and essential component for a detailed documentation of program costs and provides the potential to check comparability and transferability of those prevention programs.

Zusammenfassung Ziel der Studie: Es wird untersucht, welche Aufgaben die gesetzliche Krankenversicherung (GKV) in der Pr?vention hat, ob diese konsistent in Rechtsnormen und mit konsistenten Anreizinstrumenten umgesetzt wurden und ob die politisch beabsichtigte St?rkung der Pr?vention eingetreten ist. Methoden: Aus dem Sozialgesetzbuch V werden synoptisch die Aufgaben, Rechtsnormen und Anreizinstrumente zusammengestellt und in ihren Wirkungen und Interaktionen untersucht. Aus den Jahresrechnungen der GKV wird die Entwicklung der Ausgaben insgesamt und in den einzelnen Pr?ventionsfeldern gegen?bergestellt. Ergebnisse: Die Aufgabenzuschreibungen der GKV in der Pr?vention folgen keinem klaren wissenschaftlichen Leitbild. Sie scheinen vielmehr das sich historisch wandelnde Verst?ndnis von den M?glichkeiten der Pr?vention und zeitgen?ssige Politikans?tze ihrer F?rderung abzubilden. Gegenw?rtig f?hren unterschiedliche Rechtsnormen sowie eine Vielzahl verschiedener Anreizinstrumente zu inkonsistenten Wirkungen und Zielkonflikten. Die Pr?ventionsausgaben der GKV haben nur einen Anteil von 2?% an allen Leistungsausgaben, wovon der gr??ere Teil auf medizinische Leistungen wie Vorsorge, Schutzimpfungen und Fr?herkennungsuntersuchungen f?llt. W?hrend die Leistungsausgaben der GKV kontinuierlich steigen, haben sich die Pr?ven?tionsausgaben seit 2009 sogar verringert. Schlussfolgerung: Die derzeitigen Regelungen der Pr?ventionsaufgaben der GKV bed?rfen einer Vereinheitlichung und Bereinigung inkonsistenter Anreize. Es ist nicht erkennbar, weshalb die Ma?nahmen der Gesundheitsf?rderung, Prim?r-, Sekund?r- und Terti?rpr?vention bei vergleichbarer Evidenzbasis unterschiedlich behandelt werden sollten. Bei den Anreizinstrumenten sollte beachtet werden, dass die Ziele von Krankenkassen und Versicherte nicht deckungsgleich sein m?ssen. Anreize zur F?rderung der Pr?ventionsausgaben der Krankenkassen d?rften dort besonders wirksam sein, wo ein betriebswirtschaftlicher Nutzen der Pr?vention nicht besteht.
