Genomic Characterization of Skin and Soft Tissue Streptococcus pyogenes Isolates from a Low-Income and a High-Income Setting

Abstract: The global burden of rheumatic heart disease (RHD) has triggered a World Health Organization response to drive forward development of a vaccine against the causative human pathogen Streptococcus pyogenes . This burden stems primarily from low- and middle-income settings where there are high levels of S. pyogenes skin and soft tissue infections, which can lead to RHD.

“…The presence of superantigen genes speA, speC, speG, speH, speI, speJ, speK, speL, speM, speK/M, speQ, speR, ssa , and smeZ and DNase genes sda1, sda2, sdn, spd1, spd3 , and spd4 were determined by BLAST (100% coverage, 80% identity) with representative gene sequences against assemblies, and a manual check where needed. For other genes ( covR/S , rocA , hasABC and the nga-ifs-slo locus with promoter region), sequences were extracted from the assembled genomes and compared to the reference genome, H293 (Genbank accession NZ_HG316453.1), as previously described [28]. Antimicrobial resistance (AMR) gene carriage was determined with ABRicate (https://github.com/tseemann/abricate) using the NCBI database [29].…”

“…The lines connecting these two facets (geography and genomic lineage) indicate that although many lineages are globally dispersed, the Pacific and African geographical regions contain GAS lineages that are rarely observed in other locations. The contrasting population structure of GAS between diverse geographical settings is exemplified in preliminary findings from The Gambia 132 , Kenya 133 and remote Australia 17,134 where the circulating GAS lineages are largely evolutionarily distinct from those originating from high-income settings. One interpretation of these data is that the frequency of GAS lineages differs globally, in which there is maintenance of higher numbers of GAS genotypes from geographical regions where the disease burden is highest.…”

“…Wound swab cultures yielded S. aureus in 81% and S. pyogenes in 51% of cases. Scabies infechon (diagnosed clinically) was seen in 16% of children (95% CI [12][13][14][15][16][17][18][19][20] and was significantly associated with pyoderma [19]. Whole genome sequencing (WGS) of cultured samples demonstrated diverse emm-types [20].…”
