Ö ÖZ ZE ET T A Am ma aç ç: : Çoklu ilaç kullanımının ileri yaş bireylerde yaygın olduğu bilinmektedir. Bu çalışma; çev-resel ve kişisel hangi yaşam unsurlarının bu durumu etkileyebileceğini incelemek amacıyla yapılmıştır. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Olgu serisini tanımlayıcı, kesitsel tipte olan bu çalışmada; Kars ve İzmir ilinde Aile Sağlığı Birimi'ne kesin kayıtlı 65 ve üzeri yaş grubu 187 kişi incelenmiştir. Bağımlı değişken olan polifarmasi, beş ve üzeri ilaç kullanımı olarak değerlendirilmiştir. Bireylerin yaşam ve sağlık durumlarını inceleyen veri toplama formu yüz-yüze görüşülerek uygulanmıştır. Beden kitle indeksi (BKİ) görüşme sırasında belirlenmiştir. B Bu ul lg gu ul la ar r: : Çalışma grubunda %15,0 oranında polifarmasi saptanmıştır. Kars ilinde bu oranın % 23,0, İzmir ilinde % 8,0 olduğu görülmüştür. Cinsiyet, yaşam ortamı ve geçmiş fizik aktivite durumu ile polifarmasi arasında anlamlı ilişki bulunmamıştır. Yaş, öğrenim durumu, ekonomik durum algısı, BKİ ve geçmiş kilo durumu ile polifarmasinin ilişkili olduğu görülmüştür. Ayda bir kezden daha sık hekime başvuran ve ilaç kullanım süresi 15 yıldan uzun olanlarda, polifarmasi yüksek oranda görülmüştür. Yaşıyla orantılı olarak fizik aktivitesi olan bireylerde polifarmasi, olmayanlara göre düşük oranda izlenmiştir. İyi bir yaşam sürdüğünü düşünenlerin %8,3'ünde polifarmasi izlenirken, yaşamlarından hoşnut olmadıklarını belirten grupta polifarmasi % 30,9 oranında görülmüş, ve fark istatistiksel olarak anlamlı bulunmuştur. Kafa karıştırıcı unsurlara göre düzeltildikten sonra, Kars ilinde yaşayanlarda polifarmasi riski İzmir ilinde yaşayanlara göre 3,43 kat yüksek bulunmuştur (%95 güven aralığı =1,426-8,262). S So on nu uç ç: : Bu çalışmada çevresel yaşam koşullarının, geriatride polifarmasi yönünden etkili olabileceğini düşündüren sonuçlar elde edilmiştir. International Journal of Case Reports and Images (IJCRI) is an international, peer reviewed, monthly, open access, online journal, publishing high-quality, articles in all areas of basic medical sciences and clinical specialties.Aim of IJCRI is to encourage the publication of new information by providing a platform for reporting of unique, unusual and rare cases which enhance understanding of disease process, its diagnosis, management and clinico-pathologic correlations. IJCRI publishes SUMMARYMetabolic syndrome is made up of a cluster of conditions including obesity, impaired insulin sensitivity, hypertension and altered lipid profile. These conditions are also considered as risk factors for type 2 diabetes and cardiovascular disease. Occurrence of these conditions is on the rise worldwide despite of the increasing number of drugs used. There is evidence from the literature indicating that intervention with low carbohydrate diets, including the paleolithic diet, may be beneficial in the metabolic syndrome. Except for one study with the paleolithic diet these investigations are short in duration. Another limitation is that they do not give clues how to deal with medications patients are treated with. Third, in these studies the paleolithic diet did not produce ketosis which may limit the clinical effectiveness. Here we present a case of a patient with metabolic syndrome and excessively medicated. The patient shifted toward the paleolithic ketogenic diet and was able to discontinue medication, lost weight and her glucose parameters markedly improved. Her hypertension normalized. Currently she is on the diet for 22 months and show good adherence as also confirmed by laboratory tests. She is free of symptoms and no side-effects emerged. Previously we reported cases with epilepsy and type 1 diabetes successfully treated with the same diet. We suggest that a dietary intervention with the paleolithic ketogenic diet may be a simple, feasible and cost-effective method in some forms of chronic disorders in highly motivated patients.(This page in not part of the published article.)

Introduction:The most common belief in traditional medicine is necessarily needed to prescribe a drug. Studies show two-thirds of medical examinations are ended up with the prescription. In this study, our aim is showing the effects and features of polypharmacy on chronic disease patients and assess the sociodemographic properties, create awareness about them. Method: This descriptive cross-sectional study is done with 340 patients who admit Inonu Unıversity Turgut Ozal Medicine Centre cardiology, pulmonary disease, neurology, family medicine and psychiatry policlinics. Statistical analysis is done with SPSS ver. 22.0 and in all tests p<0.05 level is accepted as statistically meaningful. Results: In our study, there are 333 participants and 154 of them are female (46.2%) 179 of them are male (53.8%) and mean age is 52.37. Eighty participants are 65 years old or over. When we evaluate the chronic disease patients, the most common diagnoses are heart disease which is seen in 177 (52.1%) of participants and hypertension which is seen in 176 of participants (51.8%). 184 of participants (54.1%) are using less than 5 drugs and 149 of participants (43.8 %) are using more than 5 drugs. In the comparison of multidrug using and suffer from side effects, 50 of less than 5 drugs using participants (27.2%) and 29 of more than 5 drugs using participants (19.5%) had adverse effects, this difference is not statistically meaningful (p=0.100). Conclusion: We are in thought of family physicians can lower multi-drug using rates collaborating with patients for managing both acute and chronic diseases in our country where the cost of drugs are rising day by day. Because of that sociodemographic properties and drugs should be questioned for all the policlinics appeal of chronic disease patients and the cessation of drugs should be done according to the proper guidelines, they should be warned about drug interaction and how to use it.
