Gastrointestinal Bleeding and Endoscopic Outcomes in Patients with SARS-CoV-2

Abstract: Over the past year, the novel coronavirus has been a topic of significant research. Multiple gastroenterological symptoms have been associated with this infection, in addition to the well-established pulmonary presentations. Gastrointestinal bleeding can be a complication of infection by severe acute respiratory syndrome coronavirus-2, which can be exacerbated by the anticoagulants used to treat its thrombotic sequelae. We describe the clinical cases of four patients infected with the novel coronavirus, with s… Show more

“…After taking into account the title and abstract, 902 articles were further eliminated, leaving 87 papers that could be considered plausible for this analysis after passing the article review. Based on thorough review and inclusion criteria, 21 articles totaling 26,933 COVID‐19 were eventually included in this meta‐analysis 6,11–30 . Table 1 summarises the baseline characteristics of the studies that were included.…”

“…EGD of the four cases revealed variable degrees of ulcerations in the gastric mucosa or esophagus, and endoscopic intervention was required in three patients. 18 In comparison with our cohort, anticoagulation was used in all admitted COVID-19-infected patients per our local protocol. It is considered a therapeutic option for COVID-related hyper-coagulation and at the same time may simultaneously exacerbate the risk of GI bleeding in vulnerable patients.…”

“…6 There are a small number of studies on the management of UGIB in COVID-19-positive symptomatic patients, and these studies recommend conservative treatment instead of endoscopy. [16][17][18][19][20] In a study by Martin et al, only onethird of the COVID-19-positive patients with UGIB underwent endoscopy, and the remaining patients received conservative treatment. 21 Cavaliere et al suggested that COVID-19-positive patients with UGIB should be followed primarily with conservative treatment in the first 24 hours, and endoscopy should be performed in case of no response.…”
