Introduction: Gastritis is a very common and widely distributed condition worldwide. It represents one of the most common pathological entities in gastroenterology and digestive endoscopy. Our objective was to determine the sociodemographic, clinical, endoscopic, and histological aspects of gastritis in the digestive endoscopy unit of the General Hospital Idrissa Pouye (GHIP). Materials and Method: This was a retrospective study over a period of 4 years (from 1 January 2014 to 31 December 2017) at the digestive endoscopy unit of GHIP. We had collated oesogastroduodenal endoscopy (EGDE) reports with gastritis appearance with gastric biopsies and reports with normal stomach appearance with gastric biopsies and their histological reports. We collected and analyzed data on age, gender, indications for endoscopy, endoscopic findings and histological results. Results: The reports of 593 patients were analyzed. The mean age was 45 years old (range 8 -88 years old) and the sex ratio was 0.63 (230 men). The indications for endoscopy were epigastralgia in (91%) of cases, dyspepsia in (22%) of cases, pyrosis in (12%) of cases. The endoscopic appearance was normal in 229 patients (39%). The endoscopic location of the gastritis was antral in 76%, fundic in 22% and pangastric in 2%. The gastritis was erythematous in 327 patients (90%), erosive in 126 patients (35%), congestive in 53 patients (15%), pseudonodular in 14 patients (4%) and atrophic in 10 patients (3%). Histology was normal in 8 patients (1.3%) and showed gastritis in 585 patients (98.7%).

In Western countries, the current trend is to use sequential quadruple therapy or bismuth-based instead of triple therapy for the eradication of Helicobacter pylori (H. pylori). In sub-Saharan Africa, high prevalence area of the H. pylori infection, the effectiveness of these triple therapies widely used in routine has been little evaluated. The purpose of this study was to evaluate and compare the effectiveness of three patterns of first-line triple therapy based on combining a proton pump inhibitors (PPI), and 3 types of antibiotics: omeprazole (O), amoxicillin (A), clarythromycin (C) and metronidazole (M). Patients and Methods: This is a randomized clinical trial opened on 3 parallel arms: OAM (group 1 or G1), OAC (group 2 or G2) or OCM (group 3 or G3). The primary endpoint was H. pylori eradication rate after seven days triple therapy. H. pylori diagnosis infection was based on bacterium detection on the histological examination of the gastric biopsies. Histological control was performed 4 weeks after the end of treatment to assess H. pylori eradication rate. Results: The average age of our 153 patients included in the study (86 men) was 44.33 ± 11.72 years. The main reason of the endoscopy was the dyspeptic syndrome (75.16%). The gastroscopy was normal in 28.76%. A Gastric or duodenal peptic ulcer was found in 17% of cases and gastropathy in 45.75%. Histologically, the GC was active in 90.9% of cases, follicular in 35.3% of cases, atrophic in 22.5% of cases and was associated with intestinal metaplasia (IM) in 5.2% of cases. Patients of these three groups (n = 64 for G1, n = 56 for G2 and n = 33 for G3) were comparable for age, gender, endoscopy indications, alcohol consumption history or smoking, and A. S. Doffou et al. 201 anti-inflammatory drugs taking. Approximately 23% of patients experienced adverse reactions. The overall H. pylori eradication rate was 22.3%. There was no significant difference H. pylori eradication rate depending on the treatment used (28.1%, 21.4% and 15.1% for G1, G2 and G3, p = 0.34). Conclusion: The H. pylori eradication rate was poor regardless of the triple therapy used. It is desirable in the absence of bacteriological data on the primary and secondary resistance levels to optimize the eradication rate advocating the use of quadruple therapy at outset in first-line.

Le cancer de l'estomac est le deuxième cancer le plus fréquent au Mali. Objectifs : C'était de décrire les aspects épidémiologiques et diagnostiques du cancer de l'estomac. Méthodologie : Notre étude rétrospective de janvier 1999 à décembre 2008 a porté sur tous les malades opérés pour cancer gastrique dans le service de chirurgie générale du centre hospitalier universitaire (CHU) Gabriel-Touré de Bamako. Résultats : Nous avons enregistré 342 cas de cancer gastrique, soit 53,7 % des cancers digestifs. Deux tiers des malades étaient du sexe masculin. L'âge moyen était de 51,8 ans ; l'écart-type de 12,9 ; les extrêmes : entre 24 et 98 ans. Le mode de conservation alimentaire, la consommation de poisson fumé et du tô avec potasse ont été les facteurs de risque suspectés. Les signes cliniques étaient l'épigastralgie (91,4 %) ; les vomissements (81,6 %) ; l'amaigrissement (96,8 %) et une masse épigastrique palpable chez 30,4 % des malades. Chez 316 malades (94 %), le cancer était diagnostiqué au stade TNM III et IV. Le siège antropylorique a été plus fréquent, 298 cas (87,1 %). L'adénocarcinome a été le type histologique le plus fréquent (92,7 %) ; quatre cas de lymphome malin non hodgkinien et trois cas de tumeurs stromales ont été retrouvés. Conclusion : Le cancer gastrique est le premier cancer en chirurgie digestive au Mali, son pronostic reste mauvais à cause du diagnostic tardif. Pour citer cette revue : J. Afr. Cancer 3 (2011). Method: Our retrospective study from January 1999 to December 2008 covered all patients undergoing surgery for gastric cancer in the general surgery department of the Gabriel-Touré teaching hospital, Bamako (Mali). Results: We recorded 342 cases of gastric cancer, which represents 53.7% of digestive tract cancers. Two thirds of the patients were male. The mean age was 51.8 ± 12.9 years (24-98 years). The method used for food conservation was a suspected risk factor. The clinical signs were epigastric pain 91.4%; vomiting 81.6%; loss of weight 96.8% and a palpable epigastric mass in 30.4% of the patients. In 316 patients (94%), cancer was diagnosed at stages TNM III and IV. It was most commonly located in the pyloric antrum, 298 cases (87.1%). The commonest histological type was adenocarcinoma (92.7%); There were 4 cases of malignant lymphoma and 3 stromal tumors were found. Conclusion: Gastric cancer is the most common type of cancer encountered in gastro-intestinal surgery in Mali. Its prognosis remains poor because of delays in diagnosis.
