Gastric xanthelasma: an unusual endoscopic finding

Abstract: Gastric xanthelasma is a rarely encountered finding in upper gastrointestinal (GI) endoscopy. It is characterised by yellowish-white plaque in the stomach especially in the antrum or the pyloric region. Histologically it consists of foamy macrophages in the lamina propria. It is a benign condition but its appearance mimics malignancy and it is found to be associated with various conditions, some of which are considered premalignant so, histological confirmation is necessary. We present a case of a 44-year-old … Show more

“…Gastric xanthelasma, characterized by lipid accumulation in histiocytic foam cells, was a relatively scarce endoscopic finding, with prevalence varied from 0.8% to 7% in different study population[ 1 - 3 ]. As is frequently observed in the gastric mucosa of patients with early gastric cancer, xanthelasma was assumed to be a predictive biomarker or pre-tumorigenesis change of gastric adenocarcinoma[ 1 , 4 ].…”

“…Electron microscopy demonstrated foam cells with fatty cytoplasm, as seen in Figure 4 [1-2]. Treatment involves endoscopic mucosal resection and for larger lesions, duodenal preserving surgery [3].…”

“…The aetiology, pathogenesis and clinical significance of GX in relation to other gastric diseases remain unclear [ 2 ]. It is believed that the gastric mucosa affected by certain pathological processes is more prone to develop GX and for this reason, it is mandatory to look for other accompanying premalignant conditions [ 2 ]. The close relationship between H. pylori infection and GX has been described in various studies.…”

“…Gastric xanthomas (GX) are rare, non-neoplastic incidental findings with an incidence ranging from 0.2 to 0.8% [ 1 ]. The pathogenesis and clinical significance of GX are not clear [ 2 ]. Endoscopically, these lesions usually present as well-demarcated round, yellow-white plaques.…”
