It has been associated with higher complication rates, and there is no consensus on the standardized surgical approach to revision surgery. The aim of this study was to review the revision procedures performed in our institute. M Ma at te er ri ia al l a an nd d M Me et th ho od ds s: : A retrospective review of a prospectively maintained database was performed. One hundred thirty three operations were performed for morbid obesity. Primary procedures were laparoscopic adjustable gastric banding (LAGB) in 61 patients, vertical band gastroplasty (VBG) in 24 patients, Roux-en-Y gastric bypass in 21 patients, sleeve gastrectomy in 10 patients and biliopancreatic diversion with duodenal switch in six patients. Eleven patients were reoperated due to mechanical complications or inadequate weight loss. Data including age, gender, preoperative weight, body mass index (BMI) and postoperative complications were reviewed for all patients undergoing a revision procedure. R Re es su ul lt ts s: In patients with previous VBG, failure of weight loss (3/4) and outlet stenosis (1/4) were the indications. In patients with previous LAGB, mechanical side effects were the indications of surgery. The gastrocolic fistula was the indication of surgery in patient who had been revised from VBG to sleeve gastrectomy. At the mean follow-up of 13 months, the mean BMI and percentage of excess BMI were 32.3 kg/m 2 and 37.4%, respectively. There was one stapler-line leakage. Purpose: Adjustable gastric banding (AGB) and sleeve gastrectomy (SG) are restrictive bariatric surgeries that are popular in Korea. However, patients often require further conversion surgeries because weight loss failure and surgical complications tend to occur. The aim of this study was to evaluate the feasibility and safety of conversion sleeve gastrectomy (CSG) after failed primary AGB (PAGB) or primary SG (PSG). Materials and Methods: From February 2010 to April 2016, 21 consecutive patients who underwent CSG after failed PAGB or PSG were enrolled in this study. This study was a retrospective analysis of our prospectively collected database. Demographic, intra and post-operative data were collected and analyzed. Results: Twenty-one patients were enrolled in this study. This comprised 20 women and 1 man, with an average BMI of 31.8±7.8 kg/m 2 . Eighteen patients underwent PAGB and 3 underwent PSG. The mean operative time was 243.6±76.8 minutes, and the estimated blood loss was 190.9±233.2 ml. The mean hospital stay was 4.7±1.7 days. The mean follow-up after CSG was 9.3±1.0 months. Two cases developed immediate postoperative complications: one was a stricture (Clavien-Dindo surgical complication grade II) and the other, a pleural effusion (Grade I). Conclusion: CSG is a feasible and safe treatment option after failed PAGB or PSG. Further prospective studies are required to establish the strategy for conversion operations after failed primary restrictive bariatric surgery.

Background: A low quality of life has been regarded a cue to action. Some patients with unsuccessful weight loss after laparoscopic adjustable gastric banding (LAGB) undergo a conversion of gastric banding to gastric by-pass. This study examines whether patients who did or did not undergo conversion to gastric bypass differed in quality of life before and after gastric banding. Methods: Of 34 patients with unsuccessful weight loss after gastric banding (excess body mass index loss < 25%), 14 had a conversion to gastric bypass 2 years or more after gastric banding. All patients completed the RAND-36 quality of life questionnaire before gastric banding. After gastric banding, 24 patients (70.6%) filled out the RAND-36 at 6, 12, and 24 months. Results: Pre-banding and postbanding mental quality of life was lower in the conversion compared to the non-conversion group. Conclusion: Our results suggest that a relatively stable lower mental quality of life is a signal for further intervention. The bariatric team and patients should be aware that mental health may play a role in the decision for conversion to gastric bypass. The possible mechanisms underlying this finding, and limitations of the study are discussed.
