A SO-year-old female. heterozygous for beta-thalassaemia was found to have a lytic IeSion surrounded by osteosclerotic tissue in the 1st lumbar vertebra. Aspiration of the lesion showed 100 7% atypical plasma cells. The bone marrow contained 17 % myeloma cells. Despite normal electrophoresis and immunoelectrophoresis of serum and urine, 'rouleaux' tormation was pronounced. Treatment of the serum sample with 2-mercaptoethanol and heat (56°C) disclosed an uncommon pyroglobulin. Analy5is of the ammonium sulphate precipitate of the serum by sodium-dodecylsulphate polyacrylamide gel electrophoresis revealed a 43 kD component with higher anodic mobility than normal gamma chains. Ultrafiltration column chromatography of the serum revealed a narrow spike of approximately 4 S that contained gamma heavy chain antigenic determinants in addition to normal 7 S IgG.

Gamma heavy chain disease (gamma HCD) has been found in approximately 60 patients (Franklin 1978) since its discovery in 1964 (Franklin et al. 1964). Clinical features include fever and local or generalized lymphadenopathy which may wax and wane. Patients suffer from an undue susceptibility to infection. Hepatosplenomegaly is common. Blood smears often reveal lymphocytosis with atypical lymphocytes and eosinophilia. The pathological diagnosis is lymphoma or reactive hyperplasia. The disease is often rapidly progressive and fatal. In some patients it runs a more prolonged course. The case we present is remarkable in that the patient has now been in remission for 24 months.

The neoplastic proliferation of single clones of plasma cells causes synthesis of very large amount of monoclonal immunoglobulins consisting of only one type of heavy either the gamma, alpha, mu, delta or epsilon chain or only kappa or lambda light chains. Each monoclonal immunoglobulin differs idiotypically from each other. These monoctonal immunoglobulins are also called paraproteins and are frsquentiy associated with a broad heterogeneous group of plasma cell dyscrasias. Occasionally their presence is observed in a few benign conditions and in old age. In the present review a detailed account of different types of monoclonal gammapathies are describsd.
