Background Post-systolic shortening (PSS) is a sensitive indicator of myocardial ischemia. Methods and ResultsWe have developed a tissue Doppler imaging technique that portrays PSS, and whether PSS correlates with coronary artery disease (CAD) was investigated in 186 patients presenting with chest pain and normal echocardiograms. Delays of the displacement peaks from end-systole were calculated in the apical views and displayed from green (0 ms) to red (≥100 ms): detection of diastolic abnormality by dyssynchrony imaging (DADI). CAD was judged positive by DADI when the left ventricular segments were color-coded red. Patients subsequently underwent thallium-201 myocardial perfusion single-photon emission computed tomography (n=150), coronary angiography (CAG, n=74), or both (n=37). CAD(-) was defined as negative scintigraphy test and/or no significant coronary artery stenosis by CAG. In 43 patients (23%), CAD(+) was confirmed by CAG as >75% diameter stenosis. DADI predicted CAD with sensitivity of 60%, specificity of 75%, predictive accuracy of 72%, positive predictive value of 42%, and negative predictive value of 86%. Among 74 patients who underwent CAG, sensitivity was best for the left anterior descending artery. Conclusions DADI detected the regional diastolic abnormality, which correlated with the presence of CAD in patients presenting with visibly normal wall motion. (Circ J 2009; 73: 125 -131)

Background:The term acute coronary syndrome (ACS) refers to any group of clinical symptoms compatible with acute myocardial ischemia. These high-risk manifestations of coronary atherosclerosis are important causes of the use of emergency medical care and hospitalization. We evaluated the feasibility and the acute performance of the everolimus-eluting bioresorbable vascular scaffolds (BVS) for the treatment of patients presenting with ACS. Methods and results:The present investigation was a prospective, single-centre study. Clinical outcomes were reported at the 30-day, 6-month, 1 year and 2 years follow-up. The procedural success was 100.0%. After the BVS implantation a TIMI flow 3 was achieved in all 15 patients and the postprocedure percentage diameter stenosis was 16.4 ± 8.6%. No patients had angiographically visible residual thrombus at the end of the procedure. Optical coherence tomography (OCT) analysis was performed in 8 patients (53.3%) and showed that the post-procedure mean lumen area was 7.86 ± 1.81 mm 2 , minimum lumen area 5.51 ± 1.58 mm 2 . At the 30-day, 6-month, 1 year (15 patients) and 2 years follow-up (5 patients) target-lesion failure rate was 0%. Non-target vessel revascularization and target vessel myocardial infarction were not reported. No cases of cardiac death or scaffold thrombosis were observed. Conclusion: BVS implantation in patients presenting with ACS appeared feasible, with high rate of final TIMI-flow 3 and good scaffold apposition.

В диагности-ке ИБС этот метод позволяет неинвазивно выявлять ише-мию миокарда, детализировать бассейн стенозиро-ванной коронарной артерии, оценивать функцио-нальную значимость поражения коронарного русла. Главной предпосылкой, лежащей в основе метода, яв-ляется тот факт, что возникновение ишемии миокар-да сопровождается нарушением сократительной функ-ции левого желудочка. Длительное снижение или полное прекращение коронарного кровотока приво-дит к развитию острого инфаркта миокарда. Если же нарушение кровоснабжения миокарда носит прехо-дящий характер, то появляющееся патологическое движение стенки левого желудочка служит маркером для определения локализации и выраженности ише-мии миокарда. Нарушение сократимости левого же-лудочка развивается непосредственно после снижения кровотока и, как правило, до появления характерных изменений на электрокардиограмме (ЭКГ) и присту-па стенокардии. Этот ишемический каскад известен уже несколько десятилетий. Впервые его описали R.Tennant и C.J. Wiggers в 1935 году [1]. Приблизительно в это же время A.M. Master разработал стандартный протокол оценки функциональной способности сердца в ответ на физическую нагрузку с одновременным монито-рированием ЭКГ и гемодинамических параметров. Та-ким образом, современная стресс-ЭхоКГ является со-единением этих двух исследований, каждому из ко-торых более 60 лет.В качестве различных видов нагрузки при прове-дении стресс-ЭхоКГ используются:-физическая нагрузка (горизонтальная и верти-кальная ВЭМ, тредмил-тест, изометрическая нагрузка); -электрическая стимуляция предсердий (чрес-пищеводная и прямая -при имплантации ЭКС); -фармакологическая нагрузка (проба с добута-мином, дипиридамолом, аденозином, эргоновином, АТФ, комбинированные пробы); -другие виды нагрузки (холодовая, ментальная, на-грузка гипервентиляцией).Наиболее часто в клинической практике исполь-зуются горизонтальная ВЭМ, тредмил тест, чреспи-щеводная стимуляция предсердий, а также фарма-кологические пробы с добутамином и дипиридамо-лом.
