Functional Outcome After Restorative Proctocolectomy in Pigs: Comparing a Novel Transverse Ileal Pouch to the J-Pouch and Straight Ileoanal Anastomosis

Abstract: The function of ileoanal reservoirs after proctocolectomy may result from the disruption of properistaltic waves after pouch formation. The mechanism of peristalsis disruption is independent of the in situ volume of the pouch.

“…This may be explained by the overwhelming effect of the pouch itself in the short-term observation period of the present study. However, a long-term effect comparable to observations regarding transverse coloplasty [9] with an increased delay is likely based on clinical and experimental evidence concerning this technical modification [9,10]. In addition, the largest postoperative weight gain was observed in the transverse jejunoplasty group.…”

“…The transverse intestinoplasty procedure was introduced approximately 10 years ago as a reconstruction method following low anterior resection of the rectum [9]. Recently, this technique has been experimentally applied in an ileoanal pouch model and has shown comparable advantages [10]. However, studies on the use of this transverse intestinoplasty procedure during surgical reconstruction after gastrectomy have not been published.…”

“…Pig models have long been used in research on restorative proctocolectomy for the assessment of inflammatory manifestations, bacteria colonization or post-operative peristalsis in the digestive tract [10,11]. These models allow for the reconstruction of surgical procedures in humans but they are relatively rarely used due to their sizes and related costs, as well as other restrictions,.…”
