BackgroundOptical caries detection has the potential to be incorporated in telehealth medicine for preventive dental screening. The objective of this study was to evaluate and compare visible and near infrared detection methods for identifying early non-cavitated ex vivo occlusal demineralization.MethodsSix blinded examiners were used to compare the accuracy of the following three examinations in detecting occlusal demineralization: Midwest Caries ID™ (MID), visual photographic examination (CAM) and Cross Polarization Optical Coherence Tomography (CP-OCT). For each diagnostic method, two examiners assessed the extracted tooth samples 1–2 weeks apart. Teeth were then sectioned and lesion depth was confirmed (n = 42) by a blinded histological examination using a glycol based caries indicator dye. The sensitivity (Sen), specificity (Sp), Intraclass Correlation Coefficient (ICC), and Area under the Receiver Operator Curve (AUC) were calculated.ResultsFor detecting any demineralization versus sound pit and fissure enamel, the mean Sen/Sp found was 46.9/85.0 for MID, 80.5/52.5 for CAM, and 83.4/45.0 for CP-OCT. For detecting non-cavitated demineralization that progressed into the dentin, the mean Sen/Sp found was 17.3/88.0 for MID, 48.0/57.8 for CAM, and 44.2/72.7 for CP-OCT. AUC values were statistically significant (P < 0.05) in three out of four examiner assessments when MID and CP-OCT were used to detect any demineralization. AUC values were significant for a single CAM examination. When assessing deeper non-cavitated lesions, none of the assessment methods were able to yield AUC values that were significantly different than a random ‘coin flip’ test. When examining reliability, MID demonstrated the highest ICC score (0.83) and CP-OCT had the lowest (0.49).ConclusionAlthough MID and CP-OCT were useful in detecting the presence of demineralization, examiners were not able to utilize these devices to adequately assess the depth of the demineralization. This study found that MID and CP-OCT did not have markedly superior diagnostic values from simple CAM assessment for use in teledentistry.

ResumoIntrodução: O selamento de superfícies oclusais como procedimento efetivo na prevenção de cárie, desde o seu reconhecimento pela ADA em 1976, vem sendo utilizado como método preventivo, seguro e eficaz. Objetivo: O presente estudo tem como objetivo a análise da capacidade vedante de produtos utilizados como selantes de cicatrículas e fissuras de dentes humanos, cujo quesito observado é a propriedade de penetração, visualizada em microscópio eletrônico de varredura (MEV). Material e método: Realizaram-se selamentos de 40 pré-molares, sendo separados em dois grupos de 20 dentes cada um. Em um dos grupos, foi utilizado selante fotopolimerizável Fluroshield  e, em outro, cimento ionomérico Vidrion R  , mantendo-se dez dentes restantes como controle. Após executados esses procedimentos, os dentes foram cortados transversalmente por serra de precisão, passaram por etapas de fixação, além da limpeza de restos residuais, sendo então observados em MEV para avaliar o selamento e possíveis falhas ou fendas decorrentes. Resultado: O selante resinoso fotopolimerizável apresentou maior penetração e vedamento em 71,5% das amostras, atingindo a região profunda do sulco, enquanto o cimento ionomérico, utilizado como selante, apresentou maior quantidade de falhas e fendas, e em nenhuma das amostras atingiu a região mais profunda do sulco. Conclusão: O selante resinoso possui melhores propriedades nos quesitos analisados, tanto no selamento quanto na penetração.Descritores: Selante; cimentos de ionômeros de vidro; selantes de fossas e fissuras. AbstractIntroduction: The sealing of occlusal surfaces as an effective procedure in the prevention of dental caries, its recognition since 1976 by ADA, has been used as a preventive, safe and effective method. Objective: This study aimed at analyzing the products used as pit and fissure sealant of human teeth, which noted concern, is the property of penetration and porosity viewed in the scanning electron microscope (SEM). Material and method: There was sealing of 40 pre-molars, previously submitted to prophylaxis with pumice stone and water and separated into two groups of twenty teeth each. In one of the groups was used sealant polymerized by light Fluroshield  and in another, glass ionomer cement Vidrion R  and ten remaining teeth as control. After those procedures are implemented, the teeth were cutted transversely by precision saw, samples were cleaned, fixed and then observed in SEM. Result: As a result, the resin sealant fotopolimerizável showed greater penetration in 71.5% of the samples, reaching the region's deep fissure, and better adherence and less porosity, while the glass ionomer cement, used as a sealant, had higher porosity and in none of the samples struck the fissure deeper region. Conclusion: The resin sealant has better properties in the questions discussed.

SUMMARY Replacement of dental restorations has been the traditional treatment for restorations that are defective. In this five-year randomized clinical trial, restorations with localized marginal defects were treated with sealants. Thirty-two patients (mean age, 26.8 years) with 126 Class I and Class II restorations with defective margins (amalgam n=69 and resin-based composite n=57) were recruited. Treatment was seal with pit and fissure sealant on localized marginal defects (group A: n=43) and was compared with total restoration replacement (group B: n=40) and untreated restorations (group C: n=43) as negative and positive controls. Restorations were assessed by two examiners using the modified US Public Health Service criteria, observing five clinical parameters: marginal adaptation, roughness, marginal stain, teeth sensitivity, and secondary caries at baseline and at five years after treatment. At the five-year recall examination, 23 patients with 90 restorations (71.4% recall rate) were examined. A significant improvement was observed in the marginal adaptation of the restorations in group A compared with group B. None of the treated group showed trends to downgrade in any parameter. Tooth sensitivity and secondary caries showed a low frequency in all groups. No significant difference in marginal adaptation of the restorations was found between amalgam and resin-based composite restorations (p=0.191). This study demonstrated that marginal sealing of restorations is a minimally invasive treatment that may be used instead of the replacement of restorations with localized marginal defects.
