Psoriasis is a multifactorial disease with a strong genetic background. HLA-Cw6 is one of the most strongly associated psoriasis susceptibility alleles. It is repeatedly observed to affect disease course, phenotypic features, severity, comorbidities and treatment outcomes. To the best of our knowledge, the roles of HLA-Cw6 in psoriasis have not yet been thoroughly reviewed. The worldwide frequency of the HLA-Cw6 allele varies greatly, with it being generally higher in white people than in Asians. The allele is associated with type I early-onset psoriasis. Stress, obesity and streptococcal pharyngitis are commonly observed in HLA-Cw6-positive patients. Phenotypically, HLA-Cw6 has been found to be associated with guttate psoriasis. In addition, patients carrying the allele are more likely to have arm, leg and trunk involvement, and the Koebner phenomenon. Patients with psoriatic arthritis with HLA-Cw6 more often have early onset and tend to show cutaneous symptoms before musculoskeletal symptoms. HLA-Cw6-positive patients have been shown in several studies to be more responsive to methotrexate and ustekinumab. However, this difference in ustekinumab efficacy was only moderate in a post-hoc analysis of a pivotal phase III study. HLA-Cw6 positivity also tends to be less frequent in high-need patients who fail conventional therapy. Small studies have also investigated the role of HLA-Cw6 in remission of psoriasis during pregnancy, and with the comorbidities of photosensitivity and atherosclerosis. Given the diverse nature of psoriasis pathogenesis, as well as the difference of HLA-Cw6 positivity in different ethnic groups, more studies are needed to confirm the role of HLA-Cw6 in psoriasis.

IntroductionPsoriasis is a chronic skin disease with periods of recurrence and remission. The skin changes which are typical of this disease can have a considerable effect on the patient's psychological state, self-esteem and body image. It can also affect the patient's functioning in all areas of life and quality of life.AimThe present study characterized the patient needs to improve the quality of life in specified areas in patients depending on the severity of psoriatic changes.Material and methodsThe study was conducted in two stages on 100 patients aged from 18 to 66. A dermatological examination was conducted in stage one. Patients’ dermatological condition was assessed with the Psoriasis Area and Severity Index (PASI). Clinical and socio-economic information was collected in stage two using a questionnaire, a medical interview and a standardized questionnaire measuring quality of life, the WHOQOL-BREF.ResultsThe following factors had the greatest effect on the general quality of life and quality of health ratings in the studied sample: severity of psoriatic changes, duration of the most recent recurrence and sex. Severity of psoriatic changes had a negative effect on the patient's somatic, psychological, environmental and social functioning. Duration of the most recent recurrence had a negative effect on social functioning. Practical implications of this study allow dermatologists to determine the appropriate therapeutic intervention which improves the quality of life of these patients on the one hand, and will increase patient's involvement in the process of treatment on the other hand.ConclusionsQuality of life is more impaired by more severe chronic skin disease.

Purpose/aim: To assess the current prescription pattern, efficacy, tolerability and impact on Quality of life(QoL) in psoriasis patients. Material: An observational, prospective study of 18 months was carried out in a tertiary care teaching hospital. Newly diagnosed patients of psoriasis, aged ≥ 18 years were included in the study. The demographic, clinical profile, psoriasis area severity index (PASI) and drug utilization were recorded. Follow up was done at 15 days interval for two and a half months. Patient’s QoL was assessed using Psoriasis Disability Index (PDI). Results: A total 100 patients were enrolled in the study. At the time of diagnosis, 43 patients had PASI score ≤10 and 57 had > 10. At the last follow up 96 patients had PASI sore ≤ 10 and 4 had > 10. Mean of QoL at the time of diagnosis was 10.43 and at the last follow up 6.1. Pre and post-treatment PASI score showed direct intermediate significant correlation with pre and post-treatment QoL score. Most frequently fixed drug combination(FDC)of steroid and keratolytic agent were prescribed followed by anti-histaminics, steroids, minerals, anti-microbial agents, emollients, vitamins, H2-blockers, immunosuppressants, and NSAIDS.WHO core indicators were used to analyse drugs prescribed. Conclusions: In this study, the treatment of psoriasis is according to the “NICE” guideline. Patient’s treatment adherence increased because of the QoL improvement. Thus, the QoL tools are important measure of patient satisfaction and treatment monitoring.
