Frequency of bleb manipulations after trabeculectomy surgery

Abstract: Postoperative intervention is a frequent occurrence after modern glaucoma surgery. This requires intensive postoperative follow-up and is a labour-intensive undertaking. Despite interventions in our group of patients, IOP in the intervention group was always higher than in the group that required no intervention.

“…However, these studies evaluated eyes undergoing trabeculectomy with MMC which differs from our study where no MMC was used with the initial surgery. 20,21 Most importantly, IOP reduction at 12 months was decreased by 36% from baseline. Although 9% of patients needed AC fills, no patients developed choroidals, chronic hypotony, or hypotony maculopathy at any point during the study period.…”

“…No eyes were reported to have blebs at 3 years. Table 5 includes all postoperative interventions, defined as any procedure or process undertaken following surgery with the goal of enhancing the success of the surgical outcome [12]. The most commonly performed procedures included cataract extraction (19.1% of phakic eyes) followed by Nd:YAG goniopuncture (8.3%), Nd:YAG capsulotomy (7.3%), and conjunctival suture replacement (7.3%).…”

“…The implications of stopping anticoagulation, however, with modern trabeculecctomy surgery do not end at the time of the operation, as many patients require further postoperative interventions such as subconjunctival antimetabolite injections or needling procedures [16], which may in themselves promote bleeding.…”
