Introdução: a fragilidade é uma síndrome multifatorial de condições biológicas, psicológicas e sociais que predispõe a pessoa idosa a declínio da funcionalidade e maior suscetibilidade à vulnerabilidade. Tal condição pode ser agravada naquelas que apresentam comorbidades, destacando-se as cardiopatias. Objetivo: conhecer a prevalência de fatores relacionados à fragilidade e à mobilidade física, bem como a correlação entre esses indicadores em pessoas idosas em segmento ambulatorial. Casuística e método: estudo transversal com uma abordagem exploratória, descritiva e quantitativa que avaliou um grupo de pessoas idosas com cardiopatias crônicas, de ambos os sexos, com idade ≥60 anos em acompanhamento em um ambulatório de cardiologia. Os participantes foram separados de acordo com a classificação de fragilidade física de Fried e avaliadas a mobilidade física pelo Timed Up and Go (TUG), a força muscular por meio da força de preensão manual (FPM), quantidade muscular dos músculos da panturrilha através da circunferência de panturrilha (CP) e a fragilidade multidimensional com o Índice de Vulnerabilidade Clínico-Funcional (IVCF-20). A prevalência foi investigada por meio de frequência relativa e absoluta e, para as análises de correlação, o teste de Spearman foi utilizado. Resultados: a mediana da idade da amostra foi de 73 anos; 41 (61,2%) eram frágeis, 17 (25,4%) pré-frágeis; e 9 (13,4%) não frágeis. Com relação à mobilidade física, o grupo de frágeis apresento u fraqueza muscular FPM 15,3 kgf (1,6 – 33,3) e maior tempo necessário no TUG 16,3s (8,8 – 50,0). Há evidências de que exista correlação entre o IVCF-20 x TUG (p= <0,001; r=0,580), além de correlação negativa entre FPM x TUG (p= <0,001; r= -0,434) e IVCF-20 x FPM (p= <0,001; r= -0,378). Conclusão: há prevalência de fragilidade física em pessoas idosas cardiopatas em acompanhamento ambulatorial. As medidas de mobilidade se correlacionam entre si.

This research investigates how a company changes after the privatization process and how this change impacts its total structure. For Donadone and Sznelwar (2004), the pressures of new business owners, in this case coming out of the hands of the state to the hands of private companies, have a consequence of organizational design and the composition of power in companies. Therefore, this research focused on an exploratory study at AES Eletropaulo. This study has a central role in the Brazilian electricity sector because due to the radical changes it has been subjected since 90’s financialization. However, no research on careers construction of leaders in the Brazilian context had been made so far. As a research procedure this study combined quantitative and qualitative methods in order to analyze the careers of leaders of AES Eletropaulo, we collected the curriculum of the Board and Directors, since its emergence as a state enterprise in 1979 to the year of 2012. These curriculums were analysed with a Multiple Correspondence Analysis (MCA). In addition, this research was complemented with an interview with the former president of the company where the study was performed. As a result, we were able to observe the several changes that AES Eletropaulo went through, especially that, the leaders of this company had to find ways to rebuild their careers according to the new logic of the moment, which would have been facing financially. The capacity for reconversion is very large in these companies that the engineers end up reconverting their careers, these workers update themselves through courses and degrees, acquire new skills in the human, administrative and social areas, to become more flexible in a first moment of organizational change and reconvert themselves into engineers who work in financial areas, so they can stay in their elite positions. All this is marked and explained by the preference of hiring of engineers in the large Brazilian companies for high ranking positions.
Objective: To evaluate frailty and its relationship with prognostic markers in hospitalized patients with acute coronary syndrome. Methods: This cross-sectional study with a prospective variable analysis (prognostic markers) involved adults of both sexes aged ≥ 50 years with acute coronary syndrome. Patients with ≥ 3 of the following criteria were considered frail: 1) unintentional weight loss; 2) exhaustion (assessed by self-reported fatigue); 3) low handgrip strength; 4) low physical activity level; and 5) low gait speed. The included prognostic markers were: metabolic changes (lipid and glycemic profile), changes in inflammatory status (C-reactive protein), thrombolysis in myocardial infarction risk score, troponin level, angioplasty or surgery, hospitalization in the intensive care unit, length of hospital stay, and hospital outcome. Results: The sample consisted of 125 patients, whose mean age was 65.5 (SD, 8.7) years. The prevalence of frailty was 48.00%, which was higher in women (PR = 1.55; 95%CI 1.08– 2.22; p = 0.018) and patients with systemic arterial hypertension (PR = 2.18; 95%CI 1.01–5.24; p = 0.030). Frailty was not associated with age, cardiac diagnosis, or prognostic markers (p > 0.05). Conclusions: Frailty was highly prevalent in patients with acute coronary syndrome, affecting almost half of the sample, particularly women and patients with hypertension, irrespective of age. However, despite its high prevalence, frailty was not associated with markers of metabolic change or poor prognosis.
