A newly-recommended method for restoring large cavities is the biomimetic approach of using short fiber-reinforced composite (SFRC) as dentine-replacing material. The aim of the current review was to present an overview of SFRC and to give the clinician a detailed understanding of this new material and treatment strategy based on available-literature review. A thorough literature search was done up to December 2017. The range of relevant publications was surveyed using PubMed and Google Scholar. From the search results, articles related to our search terms were only considered. The search terms used were "short fiber-reinforced composite", "everX posterior", and "fiber-reinforced composite restorations". Of the assessed articles selected (N = 70), most were laboratory-based research with various test specimen designs prepared according to the ISO standard or with extracted teeth; only four articles were clinical reports. A common finding was that by combining the SFRC as a bulk base with conventional composite, the load-bearing capacity and failure mode of the material combination were improved, as compared to plain conventional composite restoration. In the reviewed studies, the biomimetic restoration technique of using SFRC showed promising characteristics, and therefore, might be recommended as an alternative treatment option for large cavities.

Large cavity designs and access cavities impair endodontically treated tooth fracture resistance. As the tooth's strength is known to reduce significantly after the root canal treatment, occlusal loading as a result of functions such as chewing, biting and certain parafunctional tendencies makes the endodontically treated tooth vulnerable to fracture. Hence, after endodontic treatment, it is vital to give adequate and appropriate restorative material to avoid tooth fractures. Accordingly, the choice for such restorative material should be dictated with the property of fracture resistance. The goal of this study was to conduct a systematic review and critical analysis of available data from in vitro studies examining the fracture resistance of endodontically treated teeth repaired with fiber-reinforced composites. A comprehensive literature search was performed using PubMed Central, Medline, Scopus, ScienceDirect, Google Scholar and LILACS. A manual search of the reference lists of the articles was also performed. Two reviewers scrutinized the papers for eligibility based on inclusion/exclusion criteria and extracted data. The studies were assessed for their potential risk of bias. A total of 15 studies met the inclusion criteria and were subsequently analyzed. Based on modified JBI & CRIS (checklist for reporting in vitro studies) guidelines, along with the methodology and treatment objective, we have formulated 13 parameters specifically to study the fracture resistance. The majority of the research had a moderate or high risk of bias. When compared to traditional hybrid composites, fiber-reinforced composites showed increased fracture resistance of endodontically treated teeth in the majority of investigations. On the other hand, limited evidence was found for the bulk fill composites. Moreover, moderate evidence was found for the fracture resistance of inlays and fiber posts with fiber-reinforced composites for core build up in endodontically treated teeth. No evidence could be found comparing the fracture resistance of crowns/endocrowns and fiber-reinforced composites in endodontically treated teeth. According to the research, using fiber-reinforced composites instead of conventional hybrid composites improves the fracture resistance of endodontically treated teeth. However, there was a high risk of bias in the research considered. No judgments could be reached about the superiority of one material over another based on comparisons between other core restorations.

Influence of Methodological Variables on Fracture Strength Tests Results of Premolars with Different Number of Residual Walls. A Systematic Review with Meta-Analysis

The aim of the current meta-analysis was to assess the impact of methodological variables in performing fracture strength tests of upper premolars. Medline (Pubmed), Embase and Google Scholar were screened for studies performing ex vivo fracture strength tests of intact upper premolars or premolars with 0, 1 or 2 walls lost. The outcome variable for each study was the maximum breaking load expressed in Newton (kg × m/s2). Methodological variables (i.e., simulation of the periodontal ligament, load inclination, tip position, tip diameter and thermocycling) were registered to perform subgroup analyses and meta-regression. Overall, 25 studies and 78 study groups were included in the meta-analysis. Intact premolars (17 study groups) were not significantly influenced by any of the methodological variables considered. Subgroup analysis for load inclination (30°/45° vs. 90°/150°) was significant for premolars with 0 (10 study groups), 1 (6 study groups) and 2 (45 study groups) walls lost; thermocycling was significant for premolars with 1 and 2 walls lost. A strong methodological heterogeneity across studies evaluating the fracture strength of upper premolars was highlighted, especially when 0, 1 or 2 walls were lost. Further studies are needed to standardize the methodology used in order to allow for across-studies comparisons.
