A fungal infection caused by Candida albicans leads to illness and death worldwide. This review mainly focused upon Vulvovaginal Candidiasis that is a common fungal infection seen in women, especially during pregnancy; Candida species cause diseases, particularly in immunocompromised patients. This type of infection occurred during the nonsterile environment. The different types of candidiasis mentioned in this review are Mucosal Candidiasis, Oropharyngeal Candidiasis, Vulvovaginal Candidiasis, Cutaneous Candidiasis, Invasive Candidiasis, Disseminated candidiasis, and Candidemia. New approaches like vaccination, antibodies, cytokine therapy, amphotericin-B loaded nanofiber, mucoadhesive are required to improve the result of the patients suffering from infections. New insights into the mechanism of anticandidal host response have contributed to the design of novel immunotherapy. The systemic and topical treatment is carried. The novel drug therapy is given in order to avoid the resistivity developed by fungus. Diagnosis is made by different techniques like direct examination and Culture Method. The other examination methods used are Pathogen-related tests, mannan tests, nucleic acid-based assays. Amphotericin–B loaded nanofibers and liposomes are developed and even certain plant extracts are using as an antifungal drug which is safer to use the future trends used for the study are Echinocandins, the latest class of drug used for the antifungal and drug act as a fungicidal that has low drug-drug interaction which makes first-line treatment for invasive candidiasis particularly. The different class of drugs has been used for the treatment are discussed with Azoles that are used in the treatment with a combination of the other class drugs.

Management of illness through medication is entering a new era in which growing number of novel drug delivery systems are being employed and are available for therapeutic use. Pharmaceutical research and development is currently focused on the development of drug delivery systems to target a drug to the desired organ or site with the aim of minimizing its overall distribution in the body. The oral mucosa has many properties which make it an attractive site for drug delivery but simultaneously provide several problems for researchers for effective and efficient delivery of therapeutic active agents. However, with the development of novel delivery techniques overcome several challenges. Oral mucosa delivery has many advantageslike high blood flow, rapid recovery, prevention of hepatic first-pass effect and pre-systemic elimation in gastrointestinal tract. On the other hand relatively small surface area and significant loss of drug due to swallowing and salivary flow are main limitations of buccal delivery. Different formulations including sprays, tablets, mouthwashes, gels, pastes and patches are presently used for delivery into and/or across the oral mucosa. Over the last 20 years, a wide range of formulations has been developed for buccal drug delivery systems but unfortunately few have been accomplished to be a medicine. One of the main reasons of this unsuccessful result could be the lack of the standardized methods which evaluate in vitro performance of buccal dosage forms. Thus aim of this review, to discuss the potential of buccal drug delivery and buccal dosage forms and also explore recent studies and in vitro analysesmethodology of buccal dosage forms. Oral Gel Loaded by Fluconazole‒Sesame Oil Nanotransfersomes: Development, Optimization, and Assessment of Antifungal Activity

Candidiasis is one of the frequently encountered opportunistic infections in the oral cavity and can be found in acute and chronic presentations. The study aimed to develop fluconazole-loaded sesame oil containing nanotransfersomes (FS-NTF) by the thin-layer evaporation technique to improve the local treatment of oral candidiasis. Optimization of the formulation was performed using the Box‒Behnken statistical design to determine the variable parameters that influence the vesicle size, entrapment efficiency, zone of inhibition, and ulcer index. Finally, the formulated FS-NTF was embedded within the hyaluronic acid‒based hydrogel (HA-FS-NTF). The rheological behavior of the optimized HA-FS-NTF was assessed and the thixotropic behavior with the pseudoplastic flow was recorded; this is desirable for an oral application. An in vitro release study revealed the rapid release of fluconazole from the HA-FS-NTF. This was significantly higher when compared with the fluconazole suspension and hyaluronic acid hydrogel containing fluconazole. Correspondingly, the ex vivo permeation was also found to be higher in HA-FS-NTF in sheep buccal mucosa (400 μg/cm2) when compared with the fluconazole suspension (122 μg/cm2) and hyaluronic acid hydrogel (294 μg/cm2). The optimized formulation had an inhibition zone of 14.33 ± 0.76 mm and enhanced antifungal efficacy for the ulcer index (0.67 ± 0.29) in immunocompromised animals with Candida infection; these findings were superior to those of other tested formulations. Hence, it can be summarized that fluconazole can effectively be delivered for the treatment of oral candidiasis when it is entrapped in a nanotransfersome carrier and embedded into cross-linked hyaluronic acid hydrogel.
