For avid glucose tumors, the SUV peak is the most reliable parameter for [18F]FDG-PET/CT quantification, regardless of acquisition time

Abstract: BackgroundThis study is an assessment of the impact of acquisition times on SUV with [18F]FDG-PET/CT on healthy livers (reference organ with stable uptake over time) and on tumors.MethodsOne hundred six [18F]FDG-PET/CT were acquired in list mode over a single-bed position (livers (n = 48) or on tumors (n = 58)). Six independent datasets of different durations were reconstructed (from 1.5 to 10 min). SUVmax (hottest voxel), SUVpeak (maximum average SUV within a 1-cm3 spherical volume), and SUVaverage were measu… Show more

“…Such studies employed maximum, mean and peak SUVs (SUV Max , SUV Mean , and SUV Peak , respectively) as biomarkers for prediction,12 diagnosis, and monitoring of treatment response 13. While SUV Max and SUV Mean have been widely studied, SUV Peak has recently been reported by Sher et al14 The latter is defined as the maximum of all the mean values computed from placing a spherical kernel of approximately 1.2 cm in diameter to yield a ~1 cm 3 sphere centered at each voxel within the tumor volume 15. In addition to SUV measurements, metabolic tumor volume (MTV) is another biomarker that has been reportedly shown to have prognostic significance for clinical outcomes such as the development of distant metastasis and loco‐regional recurrence 16.…”

“…18 F-FDG PET/CT is important in biomedical research and clinical diagnostics, and its application in lymphoma has already been reported [ 6 , 7 ]. There are several parameters being repeatedly discussed in recent studies, including maximum standardized uptake values (SUV max ), peak standardized uptake values (SUV peak ), metabolic tumor volume (MTV), and total lesion glycolysis (TLG), since they are believed to play important roles in the diagnosis and prognosis of patients with lymphoma [ 8 10 ]. Therefore, the National Comprehensive Cancer Network (NCCN) guidelines recommend the use of 18 F-FDG PET/CT for staging, response evaluation, and prognosis of lymphoma.…”

“…SUV and SUL (mean, max, and peak) were measured using a sphere volume of interest (VOI) ranging between 2 and 3 ml ( Figure 1 ). SUV/L max is a single-pixel value of the maximal SUV/L within the sphere, whereas SUV/L peak is the mean SUV/L within a predetermined volume of interest (VOI) of 1 ml around the voxel with the highest SUV/L in the sphere [ 4 ]. SUV/L mean is the average SUV/L value within the sphere.…”
