Follicular variant papillary thyroid carcinoma in a pediatric population

Abstract: In pediatric patients ≤ 19 years of age, FVPTCs have intermediate tumor features compared to PTC and FTC, but a similar OS and DSS. All pediatric patients with thyroid cancer require lifelong surveillance. However, FVPTCs > 4 cm may warrant closer follow-up due to an increased risk of death.

“…Consistent with these studies, we found that neither Cox regression analysis of DTC nor subgroup analysis showed any advantage of total thyroidectomy over lobectomy in terms of DSS. In addition, some studies using the SEER registry also demonstrated a lack of survival benefit from radiation therapy in patients with DTC (17,38). Consistent with such findings, we have found that radiation therapy did not translate to a significant positive outcome in all three DTC subgroups (p > 0.05).…”

“…Surprisingly, patients with FV-PTC had the highest diseasespecific and overall-cause survival rate, as indicated by the Kaplan-Meier analysis. Such a finding is inconsistent with the tumor features of FV-PTC, given that previous studies had shown that FV-PTC had intermediate features, between those of C-PTC and those of FTC (9,17,18). Although some studies had suggested that FV-PTC could be more aggressive than C-PTC (19,20), most studies concluded that the two subtypes have similar prognoses.…”

“…Previous review of the SEER registry revealed that in pediatric patients FVPTC without further subclassification had features intermediate between classic PTC and FTC, with tumor size larger than PTC but smaller than FTC, prevalence in younger patients and in those with a history of nonthyroidal malignancy, less frequent extrathyroidal extension, and lower rate of lymph node involvement than classic PTC.…”

“…In children, FVPTC has an incidence of approximately 22%, which is comparable to adults and it has been demonstrated to have intermediate tumor features between classic PTC and follicular thyroid carcinoma (FTC) . It appears that in this age group FVPTC without further subclassification portends a better outcome, with lower rates of lymph node involvement and recurrence when compared with classic PTC . However, there have been no published studies performed exclusively on pediatric patients comparing the clinicopathologic features of IFVPTC versus EFVPTC with its subtypes.…”
