Follicular Red Dots

Abstract: Background: Scalp dermoscopy plays an important role in the diagnosis of hair and scalp disorders, and specific dermoscopic patterns have recently been associated with several disorders causing noncicatricial alopecia, such as androgenetic alopecia, alopecia areata, trichotillomania, and tinea capitis. Observations: We describe the morphologic and pathologic features of a new dermoscopic pattern, referred to as "follicular red dots," that was found in scalp lesions of 5 patients with active discoid lupus eryth… Show more

“…Several dermoscopic features have been established in DLE [3]. Active lesions show “red dots”, which are erythematous, polygonal, and concentric in structure, linked to dilated vessels and extravasated erythrocytes in perifollicular distribution on pathology [4,5]. Large, yellow-brown dots correspond to keratotic plugs in the dilated follicular ostia [3,2].…”

“…Red dots form a good prognostic indicator and indicate possible hair growth by Tosti et al . [8] The yellow dots observed in DLE are larger in size and represent follicular plugging. Both of these changes were better appreciated in polarized mode.…”

“…They are more easily observed using video trichoscopy than with handheld trichoscopy [18]. The combination of large numbers of yellow dots and short growing hairs is a feature of AA incognita [22]. For the diagnosis of alopecia areata, other signs of alopecia areata should be taken into account, because isolated yellow dots may be seen in trichotillomania, hypotrichosis simplex, and even tinea capitis, as stated by Inui [23].…”
