Folate supplementation and methotrexate treatment in rheumatoid arthritis: a review

Abstract: We propose that folic acid supplements be prescribed routinely to all patients receiving MTX for the treatment of RA. We recommend a pragmatic dosing schedule of 5 mg of oral folic acid given on the morning following the day of MTX administration.

“…Most of our RA patients (74%) were treated with methotrexate, currently the standard traditional disease-modifying anti-rheumatic drug, which is a folate antagonist. As folic acid supplements (5 mg) have been prescribed routinely to all patients receiving methotrexate for the treatment of RA (Whittle and Hughes, 2004), all patients were simultaneously supplemented with 5 mg/day folic acid to compensate for the folate-depleting side effect of methotrexate. The intervention dose (100 mg/day) of vitamin B 6 is 62.5 times higher than the current Taiwan dietary reference intake (Department of Health, Taiwan, 2003) of 1.6 mg/day for men and women aged 451 years.…”

“…Although MTX was proved to be effective in the treatment of both RA and many cancers, discontinuation is common due to occurrence of its adverse effects [7] . There are many evidences that low dose of MTX can reache high concentration in bone as reported by Bologna et al, who found that MTX concentration in cortical and trabecular bone was 13 and 11.5 fold higher than plasma concentration of same drug [8] .…”

“…Moreover, high MCV value (as observed in this study) has been recognized as a possible predictor of potential hematological toxicity and should be closely monitored and regulated [29,30].…”
