Pitt‐Hopkins syndrome (PTHS) is a neurodevelopmental disorder characterized by intellectual disability, specific facial features, and marked autonomic nervous system dysfunction, especially with disturbances of regulating respiration and intestinal mobility. It is caused by variants in the transcription factor TCF4. Heterogeneity in the clinical and molecular diagnostic criteria and care practices has prompted a group of international experts to establish guidelines for diagnostics and care. For issues, for which there was limited information available in international literature, we collaborated with national support groups and the participants of a syndrome specific international conference to obtain further information. Here, we discuss the resultant consensus, including the clinical definition of PTHS and a molecular diagnostic pathway. Recommendations for managing particular health problems such as dysregulated respiration are provided. We emphasize the need for integration of care for physical and behavioral issues. The recommendations as presented here will need to be evaluated for improvements to allow for continued optimization of diagnostics and care.

Backgound: Mobile (flexible or correctable) hindfoot valgus deformity is common in children with spastic cerebral palsy (CP). It is accompanied by short lateral and long medial foot column. Patients and Methods:Eleven ambulatory children (20 feet) suffering spastic cerebral palsy (CP), two hemiplegic and nine diplegic, presenting with mobile (flexible) hindfoot valgus deformity, were evaluated neurologically, orthopaedically, and radiographically, and operated upon in the National Institute of Neuromotor System between September 2012 and September 2013. Double column foot osteotomy with medial cuneiform closing-wedge and cuboid opening-wedge without attacking the calcaneus was performed in all of them. Results:The results were followed-up clinically and radiographically over a period ranging from a year and half (18 months) to two years (24 months) with an average of a year and 9 months (21 months), and were graded into four categories as excellent, good, fair, and poor according to the total calculated score. According to the suggested grading system, there were 8 excellent results, 8 good results, 4 fair results, and no poor results. Conclusion:Double column foot osteotomy shortening the medial foot column and lengthening the lateral foot column to correct moderate to severe hindfoot valgus in ambulatory children with spastic cerebral palsy (CP), compared favourably with similar series, and offered option for achieving foot alignment, improving pain and skin problems and avoiding the problems associated with arthrodesis. Level of Evidence:The study is type IV therapeutic level of evidence.

StreszczenieWstęp: Za najważniejszy w kształtowaniu się stopy uznaje się wiek przedszkolny i wczesny-szkolny. Celem pracy była ocena rozwoju stóp u dzieci w wieku przedszkolnym oraz ocena ich ukształtowania z uwzględnieniem wieku i płci. Celem dodatkowym była próba ustalenia, czy za pomocą prostych metod badawczych dostępnych fizjoterapeutom można rzetelnie wyznaczać parametry stóp u dzieci.Materiał i metody: Badania zostały przeprowadzone wśród 95 dzieci przedszkolnych (49 dziewcząt-51,6% oraz 46 chłopców-48,4%). Analizie poddano następujące dane: płeć, wiek kalendarzowy dziecka, dane antropometryczne (wysokość ciała liczona w cm i masa ciała liczona w kg). Badania dokonano metodą plankturograficzną. Analizie poddano dłu-gość i szerokość stopy, kąt koślawości palucha ALFA, kąt szpotawości palca małego BETA, kąt piętowy GAMMA, wskaźnik kątowy Clarke'a oraz wskaźnik Wejsfloga.Wyniki: Badanie za pomocą metody plankturograficznej umożliwiło dokładną i rzetelną ocenę stóp grupy badanej. Potwierdzono wysoce istotną statystycznie różnice pomiędzy szerokością i długością stopy prawej i lewej, a wiekiem dzieci. W przypadku wskaźnika Wejsfloga odnotowano wysoce istotne statystycznie różnice pomiędzy wynikiem pomiaru u dzieci 5 -letnich względem dzieci 3-letnich i 4-letnich. Nie stwierdzono różnic pomiędzy budowa stóp dzieci biorąc pod uwagę ich płeć.Wnioski: 1. Budowa stóp dziewczynek i chłopców w okresie ich progresywnego rozwoju kształtuje się w sposób zbliżony do siebie. Jedynie dzieci 5-letnie wykazują wyższy poziom rozwojowy wysklepienia stóp w porównaniu do pozostałych dzieci. Nie zaobserwowano istotnych różnic pomiędzy rozwojem stóp u dziewcząt i chłopców, co świadczy o braku wyraźnie zaznaczonego dymorfizmu płciowego w kształtowaniu się wysklepienia stopy. 2. Większość dzieci miało prawidłowo zbudowane stopy, zatem zlecanie ćwiczeń korekcyjnych dla wszystkich dzieci nie znajduje swojego uzasadnienia. 3. The following data were analyzed: sex, age, anthropometric data. The research was conducted by foot print method.
