First series of left ventricular assist device exchanges to HeartMate 3

Abstract: LVAD exchanges from HM II as well from HVAD to HM 3 are proven to be technically feasible. Due to the advantages and technical improvements of the new-generation pumps, this procedure is an excellent opportunity to give patients access to a superior generation of assist device.

“…Dealing with chronic DLI can be complicated. If antibiotic therapy fails, an exchange of the LVAD remains the inevitable alternative (15)(16)(17). Altogether using a double tunneling technique for driveline implantation seems to be an important factor to lower infection rates in LVAD patients.…”

“…LVAD device exchange is an invasive surgical procedure that does not come without risks. The overall early postoperative mortality for patients undergoing exchange appears to be low (25,26). There have been reports of postoperative bleeding and recurrence of underlying etiology that necessitated the exchange (such as infection and thrombosis), although the true rates of postoperative adverse events have not been elucidated in the literature (25,27).…”

“…Even though the need for LVAD exchange is a hazardous complication in VAD therapy, it can be used to upgrade a patient to a new generation assist device e.g., HeartMate 3 (27)(28)(29)(30). The exchange procedure offers an opportunity to upgrade patients to a new generation pump which offers e.g., advanced reduction of adverse events or longer battery capacities (27).…”
