First results of cataract surgery and implantation of negative power intraocular lenses in highly myopic eyes

Abstract: Cataract surgery can be performed in highly myopic eyes without intraoperative complications. A posterior chamber IOL should be implanted for postoperative refraction and intraocular stability, even if negative lens power is required.

“…1 The increasing popularity of clear lens extraction has brought increased expectations of predictable postoperative refraction with IOLs used in refractive surgery for high myopia. 2 However, precise biometry prediction in extremely long eyes requiring concave lenses has always been difficult, 3 although the SRK/T 4 (Appendix) is probably the most accurate formula and is now widely used. One biometry study 5 including 40 eyes with axial lengths between 27.0 mm and 35.0 mm showed greater accuracy in high myopia with the SRK/T formula than with the SRK II, Holladay, Hoffer, or Binkhorst II formulas.…”

“…Previous studies showed a high incidence of PCO and subsequent neodymium:YAG (Nd:YAG) laser posterior capsulotomy in eyes with high myopia. [4][5][6] However, it has also been shown that the occurrence of PCO in highly myopic eyes was less if a low-power or zero-power intraocular lens (IOL) was implanted. 7 The purpose of our study was to examine whether the degree of myopia and axial length are correlated with the occurrence of PCO after phacoemulsification surgery.…”

Kaschube [6] (76 Augen; Kontrollzeit durchschnittlich 17 Monate) fand in 39,5% and Kohnen [8] (32 Augen; Kontrollzeit 6-36 Monate) in 43,8% einen Nachstar.
