Introduction: Careful monitoring of the hypercoagulable state is required during pregnancy. However, coagulation and fibrinolysis markers are not fully utilized because there are no reference values reflective of coagulation and fibrinolysis dynamics during pregnancy, which differ from the nonpregnant state. Methods: Changes in antithrombin (AT), fibrinogen (Fbg), prothrombin fragment 1þ2 (F 1þ2 ), thrombin-antithrombin complex (TAT), soluble fibrin (SF), D-dimer (DD), and protein S (PS) were investigated in healthy pregnant women, and reference ranges in the early, mid, late, and end stages of pregnancy were established. Results: The AT was essentially constant throughout pregnancy. The Fbg, F 1þ2 , TAT, and DD increased significantly as pregnancy progressed. In contrast, SF did not show a significant increase throughout the entire pregnancy period. Total PS antigen and total PS activity showed a corresponding decrease from early gestation. When test data in 3 cases in which deep vein thrombosis or intrauterine fetal death occurred during pregnancy were compared to the established reference ranges, all of the cases had multiple markers with values that exceeded the reference ranges. Conclusion: Establishing reference ranges for each week could potentially make it possible to evaluate abnormalities of the coagulation and fibrinolysis systems during pregnancy. Of note, SF might be a useful marker that reflects thrombus formation during pregnancy. Larger-scale studies will be required to establish reference ranges for every gestational week.

У женщин с предрасположенностью к тромбозам (с тромбофилией) во время беременности развивается патологическая гиперкоагуляция, что мо-жет привести к ранним и поздним репродуктивным по-терям. К наиболее значимым полиморфизмам генов тромбофилии относятся дефицит антитромбина III, про-теина С, мутация Лейден, наследственная гипергомоци-стеинемия и мутации некоторых других факторов свер-тывания. Кроме того, существует группа тромбофилий, обусловленных гиперагрегацией. В настоящее время наиболее безопасными и эффективными препаратами для профилактики и терапии тромботических осложне-ний считаются гепарин и его производные. Однако оце-нить эффективность гепаринов при использовании лишь стандартных методов изучения гемостаза (активирован-ное частичное тромбопластиновое время, тромбиновое время, протромбиновое время) и маркеров внутрисосу-дистой активации свертывания (растворимые фибрин-мономерные комплексы, D-димер) невозможно в связи с недостаточной их чувствительностью. Одним из новых тестов качественной и количественной оценки коагуля-ционного состояния плазмы, способным фиксировать даже минимальные сдвиги равновесия свертывающей системы, считается исследование тромбодинамики. Цель. Оценить целесообразность применения теста тромбодинамики у женщин с патологией беременности в I триместре. Доказать возможность его применения как наиболее чувствительного метода контроля тера-пии низкомолекулярными гепаринами (НМГ). Методы. В исследование включены 23 беременные с акушерско-гинекологической патологией и/или тром-ботическими осложнениями в анамнезе и угрозой пре-рывания беременности в I триместре. Женщины были в возрасте 22-38 лет (медиана 30 лет). Интегральная оценка системы гемостаза проводилась с помощью те-ста тромбодинамики. Результаты. Терапия НМГ под контролем тромбодинами-ки проведена у 20 из 23 женщин. В результате статисти-чески значимо изменялись только показатели тромбоди-намики (p < 0,05). У 14 женщин беременность закончилась родами здоровыми детьми на сроках 38-40 нед. (все па-циентки в I триместре получали НМГ). Заключение. Тест тромбодинамики -наиболее надеж-ный метод контроля терапии НМГ, поскольку позволя-ет фиксировать даже минимальные сдвиги равновесия свертывающей системы.
