Antepartum nonstress fetal heart rate (FHR) monitoring may become a valuable screening tool for all pregnancies. Two methods proposed for an antepartum FHR monitor are examined: abdominal electrocardiogram (AECG) and variable range directional Doppler (RDD). A good quality (detailed, clearcut baseline) RDD FHR record was obtained in 74/75 (99%) examinations, and a good quality AECG record was obtained i31/75 (41%). Each patient was monitored in the supine, lateral and sitting positions. Considering all positions, 76% of the RDD records and 19% of the AECG records were good. The RDD system was efficacious for routine, clinical antepartum FHR monitoring, but the performance of the AECG was too limited for this purpose.

SUMMARY The study of fetal heart rate by fetal electrocardiography has brought into focus the variable aetiology of fetal bradycardia. It has emphasised that different patterns of bradycardia may have a different basis with a different prognosis and require different treatment. Some cases, which do not fit into any group, are not explicable by current theory. For continuous monitoring or research purposes fetal electrocardiography is more satisfactory than phonocardiography. It requires the use of abdominal electrodes during pregnancy and scalp electrodes in labour. Automatic recording of fetal heart rate may be obtained from the latter, though no equipment suitable for clinical use is yet available. Study of fetal electrocardiographic complexes with sophisticated equipment requires large and expensive resources, which are not likely to become available in the U.K. RÉSUMÉ Électrocardiographic fetale; Une revue de la littérature L'étude de la vitesse du coeur par électrographie fetale a mis au point l'étiologie variable de la bradycardie fetale. Elle a fait valoir que différents types de bradycardie peuvent avoir des bases différentes avec un pronostic différent et demandent un traitement différent. Certains cas, qui ne se rangent dans aucun groupe, ne sont pas explicables par la théorie en cours. Pour un contrôle continu ou pour la recherche, l'électrocardiographie fetale est plus satisfaisante que la phonocardiographie. Elle exige l'emploi d'électrodes abdominales pendant la grossesse et d'électrodes sur le cuir chevelu pendant le travail. l'enregistrement automatique de la vitesse du coeur du fetus peut être obtenu à l'aide de ces dernières électrodes, mais il n'y a pas encore d'équipement adapté a l'usage clinique. l'étude des complexes d'électrocardiographic fetale au moyen d'un équipement artificiel exige des ressources financeères qui vraisemblablement ne seront pas disponibles en Grande‐Bretagne. ZUSAMMENFASSUNG Fötale Elektrokardiographie: Eine Literatur‐Übersicht Das Studium der fötalen Herzschlaggeschwindigkeit durch fötale Elektrokardiographie hat die veränderliche Ätiologie der fötalen Bradykardie zur Aufmerksamkeit gebracht. Es hat betont, dass verschiedene Bradykardie‐Muster eine verschiedene Grundlage mit einer verschiedenen Prognose haben können, die verschiedene Behandlung erfordern. Einige Fälle, die in keine Gruppe passen, lassen sich anhand allgemein bekannter Theorie nicht erklären. Fur fortdauernde Kontrolle oder Forschungszwecke erweist sich fötale Elektrokardiographie zufriedenstellender als Phonokardiographie. Es erfordert die Verwendung abdominaler Elektroden während der Schwangerschaft und Kopfhaut‐Elektroden während der Entbindung. Automatische Aufzeichnung der fötalen Herzschlaggeschwindigkeit kann durch die letzteren erhalten werden, obwohl noch keine Geräte die für klinischen Gebrauch geeignet wären, erhältlich sind.
