Objectives: This study aimed to investigate the relationship between the presence of carotid arteriosclerosis (CAS) and blood pressure variability (BPV) in patients with essential hypertension. Methods:One hundred and forty four essential hypertension patients underwent ambulatory BP monitoring for 24 hours after hospitalization. Common BPV metrics were calculated. General clinical parameters, including age, gender, height, weight, history of coronary heart disease, stroke, diabetes, hypertension, smoking and drink, were recorded. Biochemical indices were obtained from a blood test. Carotid intima-media thickness (IMT) and carotid plaques were assessed to separate patients into a non-CAS group (IMT0.9 mm; n=82) and a CAS group (IMT>0.9 mm; n=62). BPV metrics and clinical parameters were analyzed and compared between the two groups.

92В настоящее время стратификация сердечно-сосу-дистого (СС) риска основана на оценке факторов риска и уровней биохимических маркеров, при этом традиционная оценка риска имеет общепризнанные ограничения, особенно в группах с более низким риском, таких как женщины, люди молодого и сред-него возраста [1][2][3]. Данные нескольких популяцион-ных исследований указывают на то, что более поло-вины СС событий происходит среди лиц, соответ-ствующих низкому или умеренному СС риску согласно широко применяемым шкалам (SCORE, Фрамингемская шкала) [4]. По данным разных иссле-дований, факторы риска объясняют только 52-66% совокупности причин, приведших к развитию атеро-склероза [5,6]. Большую долю составляет так называ-емый "необъясненный атеросклероз", развитие которого нельзя обосновать известными факторами риска [5]. При этом известно, что атеросклероз начи-нается в раннем возрасте и его распространенность и выраженность увеличиваются в течение жизни. Более половины сердечно-сосудистых событий происходит у лиц с низким или умеренным сердечно-сосудистым риском, который оценивается на основа-нии традиционных факторов риска. При этом с помощью ультразвукового исследования артерий удается выявить атеросклероз уже на ранних стадиях его развития. Высокая частота наличия субклинического атеросклероза у лиц низкого и умеренного сердечно-сосудистого риска, который одновременно является реализацией как традиционных, так и еще "необъясненных" факто-ров риска, ультразвуковые маркеры атеросклероза имеет высокий потенциал улучшить стратификацию сердечно-сосудистого риска. В данном обзоре рас-смотрены ультразвуковые маркеры атеросклероза с позиции их ассоциации с традиционными факторами риска, способности улучшать прогностическую ценность имеющихся шкал оценки риска, а также возможности влиять на исходы. Прогностическая значимость маркеров рассмотрена в рамках пер-вичной профилактики среди представителей общей популяции, среди лиц высокого риска, а также с позиции количественного индикатора степени ате-росклеротического поражения или способности выступать в качестве сурро-гатного маркера сердечно-сосудистых заболеваний. More than a half of cardiovascular events occur in low to moderate cardiovascular risk patients if assessed based on the traditional risk factors. At the same time, ultrasound examination of arteries makes it possible to reveal atherosclerosis even at early stages of its development. High prevalence of subclinical atherosclerosis in low to moderate risk patients, which is a realization of traditional and "unknown" risk factors, makes ultrasound examination a useful method for risk stratification. The review is focused on ultrasound markers of atherosclerosis with association to traditional risk factors, on the possibility to improve predictive role of current scores and influence on outcomes. Objective: There have been few reports on the frequency of atherosclerotic diseases complicating carotid artery stenosis. This study was conducted to clarify the frequency of various atherosclerotic diseases concurrent with carotid artery stenosis that required revascularization. Methods:Examinations concerning various atherosclerotic diseases were reviewed using clinical records of 152 consecutive patients who underwent carotid artery stenting from January 2012 to October 2016 at our hospital. The patients were also divided into those with systemic atherosclerosis (SA group) and those without SA (non-SA group), and their characteristics were analyzed. Results:Of the 152 patients, carotid artery stenosis was complicated by other atherosclerotic diseases in 50 (32.9%), of whom 28 (18.4%) had coronary artery disease, 23 (15.1%) had peripheral artery disease, and 3 (2.0%) had renal artery stenosis. In the SA group, the frequencies of diabetes and dyslipidemia were significantly higher, hemoglobin A1c (HbA1c) level was higher, and high-density lipoprotein (HDL) cholesterol level and estimated glomerular filtration rate (eGFR) were lower. Conclusion:Carotid artery stenosis that required revascularization was frequently complicated by other atherosclerotic diseases. It is considered necessary to treat carotid artery stenosis with the prevention of cardiovascular events.
