These guidelines for the treatment of persons who have or are at risk for sexually transmitted diseases (STDs) were updated by CDC after consultation with a group of professionals knowledgeable in the field of STDs who met in Atlanta on April 30-May 2, 2013. The information in this report updates the Sexually Transmitted Diseases Treatment Guidelines, 2010 (MMWR Recomm Rep 2010;59 [No. RR-12]). These updated guidelines discuss 1) alternative treatment regimens for Neisseria gonorrhoeae; 2) the use of nucleic acid amplification tests for the diagnosis of trichomoniasis; 3) alternative treatment options for genital warts; 4) the role of Mycoplasma genitalium in urethritis/cervicitis and treatment-related implications; 5) updated HPV vaccine recommendations and counseling messages; 6) the management of persons who are transgender; 7) annual testing for hepatitis C in persons with HIV infection; 8) updated recommendations for diagnostic evaluation of urethritis; and 9) retesting to detect repeat infection. Physicians and other health-care providers can use these guidelines to assist in the prevention and treatment of STDs.

Г енитальный герпес (ГГ) -хроническая пожиз-ненная вирусная инфекция, передаваемая половым путем, которая вызывается вирусом простого герпеса (ВПГ) 2 и/или 1 типа.Ежегодно ГГ заболевает около 500 тыс. человек. У боль-шинства из них заболевание остается недиагностирован-ным вследствие частых субклинических и атипических форм. В России заболеваемость ГГ в 2015 г. составила 13,5 случая на 100 тыс. населения: у лиц в возрасте 15-17 лет -9,4 случая на 100 тыс. населения, а в возрасте старше 18 лет -16,4 случая на 100 тыс. населения. Согласно дан-ным ВОЗ от 2015 г., в мире 536 млн инфицированных ВПГ 2 типа и 3,7 млрд инфицированных ВПГ 1 типа (67% насе-ления). В многочисленных исследованиях на общей попу-ляции показано, что женщины инфицируются чаще, чем мужчины, при одинаковом количестве половых партнеров в течение жизни. Самая высокая заболеваемость ГГ реги-стрируется в возрастной группе 20-29 лет, а второй пик заболеваемости приходится на возраст 35-40 лет [1-3].Возбудителем ГГ является ВПГ, облигатный внутри-клеточный паразит, принадлежащий к семейству Herpesviridae, подсемейству L -Herpesviridae, виду ВПГ. Данный вирус обладает пантропизмом (дерматонейро-тропный), т. е. способностью присоединяться к клеткам кожи, слизистых оболочек, центральной и перифериче-ской нервной системы, печени, эндотелию сосудов, клеткам крови: Т-лимфоцитам, тромбоцитам, эритроци-там [4].При изучении большого количества штаммов вируса были выделены 2 основные антигенные группы этих возбудителей -ВПГ 1 типа и ВПГ 2 типа, которые незна-чительно различаются по вирулентности и патогенно-сти. ВПГ 1 типа преимущественно инфицирует слизи-стую оболочку полости рта, и его распространение при заболевании полового тракта происходит преимуще-ственно генитально-оральным путем, тогда как ВПГ 2 типа обладает большей тропностью к слизистой обо-лочке гениталий.

Famciclovir, the oral formulation penciclovir,is also clinically effective in the treatment of a variety of HSV-1 and HSV-2 infections [61].Valacylovir is a valyl ester of acyclovir that has greater bioavailability [62]. A number of topical antiviral are effective for herpes labialis,includingacyclovir,penciclovir and docosano [57].…”

Herpes is caused by Herpes simplex virus (HSV).HSV-1causes orofacial and genital infections.HSV-2 causes primarily genital infections. In the U.S.HSV-1 is more prevalent than HSV-2, and HSV-2 is predominant among American African, and women. Initial HSV infection is often subclinical. Viral replication occurs in ganglia, virus spreads to other mucosal surfaces through peripheral sensory nerves. Clinical manifestation include, skin and mucosal infection, orofacial herpes, genital herpes, herpetic whitlow, herpes encephalitis, neonatal herpes acquired at the time of delivery, although rare but serious condition and herpes keratitis damage the eyes. Diagnostic tests include, culture, antibodies detection, skin biopsy, and PCR for presence of viral DNA.Topical anesthetic treatment to relieve itching and pain. Antiviral drugs such as acyclovir,valaciclivir and famiciclovir are effective. Antiviral for herpes suppressive therapy is recommended during the last months of pregnancy. Recurrent outbreaks are common and vary between individuals; genetics play a role in the frequency of cold sore outbreaks. Development of a drug to block the microRNA whose job is it to suppress HSV-1 into latency, may prevent the outbreaks. Prevention with antiviral medication in conjunction with a condom.
