False-positive rapid plasma reagin testing in patients with acute Plasmodium vivax malaria: A case control study

Abstract: Summary Non-treponemal tests such as the rapid plasma reagin (RPR) assay are mainstays of syphilis diagnosis, but false-positive tests are common. We identified false-positive RPR titers in 8.2% of patients with malaria due to Plasmodium vivax in northern Peru. Similar rates were not detected in patients with other acute febrile illnesses.

“…Unlike the prevalence of other types of RTIs at a relatively lower level, the positive rate of syphilis (0.73%) was high in our study population, and it was also much higher than the national level from 2010 to 2012 in China (0.37%) [ 18 ]. However, according to the local study site staff feedback, some of the positive syphilis cases were only based on the RPR test and were not confirmed by TPPA, indicating that our estimated prevalence of syphilis might have been overestimated with a false positive rate of approximately 5–10%, as previous studies reported [ 19 , 20 ].…”

“…To the best of our knowledge, there have only been previous case reports [11] and clinical expert guidance [2] for these outcomes, not retrospective reviews of results. Other studies also reviewed false-positive results related to the RPR [12,13], not the reverse algorithm. Our findings, in contrast, constitute what we believe is the first study to categorize inconclusive syphilis results for an entire public health unit based on the frequency of the various possible causes of such results and the risk factors associated with these outcomes.…”
