Failure of endovascular abdominal aortic aneurysm graft limbs

Abstract: Fully supported AAA endografts provide superior endograft limb patency compared with unsupported designs. Consideration should be given to routine stenting of all unsupported endograft limbs. Aortomonoiliac grafts and bifurcated grafts provide similar results for endograft limb patency. Femorofemoral bypass grafts performed in conjunction with aortomonoiliac grafts for AAA disease provide excellent short-term patency.

“…In the second case of graft limb occlusion, the patient had small common (5.2 mm) and external (6.7 mm) iliac arteries, and a possible reason for this occlusion could be that the contralateral limb device component was much oversized. Nevertheless, the limb occlusion rate of 4% (2/50) in our series is comparable with the reported incidence (3.7% to 7.7%) in larger series [7,22,35].…”

“…Others have also reported successful outcomes after secondary interventions for late EVG failures. 7,10,13,16,30 However, failure to detect late failure can lead to aneurysm rupture and death. 2,18 Therefore, diligent postoperative surveillance is critical after an EVG repair.…”

“…4). 7,10,13,[15][16][17]30,31 The method of treatment for a given endoleak is the subject of considerable controversy. Based on the findings of various experimental models, we believe that when an endoleak has a long and narrow channel, pressure transmission across thrombus induced in this channel may be significantly reduced.…”

“…6 Crossover femorofemoral bypass has been shown to be effective, with higher patency rates when combined with AUI in aneurysmal disease than in occlusive disease, with 3-year patency rates of Ͼ90% touted in several studies. [11][12][13] These studies, however, included very few patients with total chronic occlusions, and many of the patients included in these reports had undergone AUI graft placement before bifurcated EVAR was available at their institution, thus including patients who did not necessarily have significant peripheral vascular disease. In this study, we looked only at patients with occluded iliac arteries and AAAs.…”
