This study aimed to quantify the association between adequacy of prenatal care and prevalence of folic acid, iron, and multivitamin intake during pregnancy. Data were obtained on socio-demographics, prenatal care, pregnancy complications, and use of vitamin/mineral supplements for 836 women, using a postpartum interview. Associations with the use of vitamin/mineral supplements were quantified with risk ratios (RR), computed by generalized binomial regression. A high proportion of women reported the use of folic acid (81.9%), iron (55.4%), and multivitamins (76.2%) as supplements during pregnancy. Use of supplements was independently associated with adequacy of prenatal care (adequate vs. inadequate: folic acid, RR = 2.28; 95%CI: 1.58-3.29; iron, RR = 1.99; 95%CI: 1.57-2.52, multivitamins, RR = 1.97; 95%CI: 1.54-2.51). Higher schooling was also associated with increased use of folic acid (RR = 1.42; 95%CI: 1.18-1.70), but not multivitamins (RR = 0.87; 95%CI: 0.77-0.98). Use of folic acid was less prevalent in single women (RR = 0.67; 95%CI: 0.48-0.95) and during unplanned pregnancies (RR = 0.81; 95%CI: 0.71-0.92). Adequacy of prenatal care is a major determinant of vitamin/mineral intake during pregnancy.

Com o objetivo de identificar os determinantes maternos do consumo de medicamentos na gestação por classes de risco, realizou-se estudo transversal na linha de base de coorte populacional, com 2.262 mães de nascidos vivos em Passo Fundo, Rio Grande do Sul, Brasil. Coleta de dados realizada no Sistema de Informações sobre Nascidos Vivos e mediante entrevistas domiciliares (instrumento adaptado do Maternity Advice Survey). Os medicamentos foram classificados segundo a agência americana Food and Drug Administration. Realizaram-se análises bivariadas e regressão logística seguindo modelo hierarquizado de determinação. Oitenta por cento das mães consumiram pelo menos um medicamento na gestação, distribuídos nas classes de risco: A (53,4%), B (18,1%), C (24,46%), D (1,47%), X (0,06%) e V (vários) (2,1%). Pré-natal incompleto e idade materna < 20 anos determinaram o consumo de medicamentos na classe A e intercorrências na gestação nas classes B, C e D. Na classe C, cesariana e aborto prévio também determinaram o consumo, que na classe V foi independente das variáveis analisadas. Fatores maternos, especialmente intercorrências de saúde, determinam o consumo de medicamentos na gestação sugerindo que seu uso seguro e correto pode ser orientado pelo prescritor.

Pregnancy constitutes a major life event that confronts women with unique physical, social, economic and psychological adjustments. Epidemiological evidence suggests that the prevalence and severity of most common mental health disorders (such as depressive and anxiety disorders) during pregnancy are not different from those in the population of non-pregnant women. However, the mental health state of pregnant women has received considerable clinical and scientific attention because of its suggested influence on the pregnancy outcome, the baby, and the future child. There is emerging evidence that experiencing antenatal distress may lead to suboptimal birth, adverse birth outcome, This thesis begins with an outline of the research problem and sets the scene by providing the theoretical and methodological assumptions that underlie the design and the conduct of this thesis. In introducing this work, I argue that it is important to draw from a mixed-methodology scholarship grounded in feminism and critical realism to ensure a multi-faceted understanding of such complex phenomena as pregnancy and distress. I then position my research within a background of the existing literature, and pinpoint the gaps which this thesis aims to address. Starting with an epidemiological systematic review, I explore the link between antenatal distress and preterm birth, followed by a meta-synthesis of the qualitative literature that has examined the experience of antenatal distress. First, Isuggest that direct causal relationships between antenatal distress and adverse birth outcomes cannot be established in a conclusive way, and secondly I argue that such assumptions can be problematic for women as the experiences of antenatal distress involve a complex journey. I then suggest that pregnancy distress entails a process similar to that of iii grief and loss as a result of women's inability to situate their experience within the good and perfect mother discourse.The empirical section of this thesis draws on data I collected both via an online survey and through interviews and is presented in three consecutive studies. The online survey, which was completed by over 290 eligible pregnant women from Australia and New Zealand, consisted of various psychosocial questionnaires administered at three time points (twice during pregnancy and once after birth). This longitudinal data formed the basis for the first empirical study which explored the psychosocial predictors of antenatal distress. In this study I conclude that it is a woman's overall sense of coherence that predetermines her distress during pregnancy. In order to advance the understanding of the meanings that women ascribed to their experience, and how these affected their 'at odds' perceptions of motherhood, I then present the findings from a qualitative study with 18Australian women who reported pregnancy distress.
