Objective: To compare refractive outcomes using the Lenstar optical low-coherence reflectometry (OLCR) biometer in the following cases: phacovitrectomy for epiretinal membranes and macular holes (phacovitrectomy group); sequential cataract surgery after prior vitrectomy (sequential-phaco group); routine cataract surgery (phaco-only group). Methods: This study was a retrospective, consecutive comparative series. Main outcomes were mean prediction error (ME) and mean absolute prediction error (MAE). Secondary outcome was the variance in prediction error. Results: ME was significantly more myopic in the phacovitrectomy group (−0.08 ± 0.77D, mean ± SD, p = 0.04) and the sequential-phaco group (−0.09 ± 0.51D, p = 0.01) compared to the phaco-only group (+0.24 ± 0.53D). MAEs were not statistically different across the 3 groups. The phacovitrectomy group had a wider variance in prediction errors compared to the phaco-only group (p = 0.03). Conclusions: There is a myopic shift of approximately −0.3D in both phacovitrectomy and sequential-phaco cases compared to phaco-only cases when using OLCR-based optical biometry. Phacovitrectomy outcomes are more variable compared to phaco-only eyes.

Summary Statement:Combined phacovitrectomy as primary repair surgery for rhegmatogenous retinal detachment (RRD) included a small biometric error that was within the tolerable range in most cases.The biometry used for IOL power selection must be checked by comparing with the fellow eye and the known refraction, especially in macula-off RRD cases. 3ABSTRACT PURPOSE: To evaluate the accuracy of intraocular lens (IOL) power estimation in eyes having phacovitrectomy for rhegmatogenous retinal detachment (RRD).METHODS: Retrospective case series review of 100 consecutive eyes that underwent phacovitrectomy for RRD. Axial lengths (ALs) were measured using optical biometry and/or ultrasound. Achieved and predicted refraction were compared to calculate the mean postoperative refractive prediction error and the mean absolute prediction error (MAE).Factorial analysis of variance (ANOVA) models were developed to assess outcome overall and between subgroups. RESULTS: 95 eyes had postoperative refraction; 41 macula-on (43%), 54 macula-off (57%).The mean postoperative prediction error was -0.34 ± 0.89D. There was no statistical significant difference in the refractive outcomes between macula-on and macula-off groups (p>0.05). Overall, using MAE as the outcome measure, optical biometry was more accurate then ultrasound (p=0.040). However, significantly more ultrasound measured ALs were selected for IOL power estimation in macula-off group compared to the macula-on group (p=0.016).CONCLUSIONS: Combined phacovitrectomy in RRD included a small biometric error that was within the tolerable range in most cases. The accuracy of AL used for IOL power calculations must be checked by comparing with the fellow eye and the known refraction, especially in macula-off RRD cases.4

Витреоретинальная патoлoгия занимает весoмoе местo в структуре заболеваний глазнoгo яблока, явля-ясь причиной слепоты и слабовидения в Рoссийскoй Федерации в 16% случаев [1]. Кoмбинирoванные патoлoгические изменения сетчатки и стеклoвиднoгo тела (СТ) требуют кoмплекснoгo хирургическoгo ле-чения у бoльшинства пациентов [2]. Одной из наи-более востребованных операций в арсенале витрео-ретинальных вмешательств является витрэктомия. В течение последних лет показания к витрэктомии расширились в связи с появлением новых современ-ных инструментов и минимизацией послеопераци-онных рисков [3]. При многих заболеваниях, таких как отслойка сетчатки, диабетическая ретинопатия, пролиферативная витреоретинопатия, может воз-никнуть необходимость полного или практически полного удаления измененного СТ [4]. Однако, не-смотря на эффективность данного метода лечения, его применение может стать причиной ряда струк-турно-функциональных изменений различных отде-лов глазного яблока. В современной специальной литературе подробно освещены вопросы подобных нарушений заднего отрезка глаза после витрэктомии [5,6]. Однако указанные изменения, наблюдаемые в проекции пространства переднего отрезка глаза, из-учены недостаточно.В доступной литературе имеются сведения, ука-зывающие на близкую анатомическую связь между передней частью СТ и окружающими его структура-ми [7]. Витрэктомия может являться фактором, ин-дуцирующим нарушение данной связи, что в свою очередь может вызвать морфофункциональные из-менения глазного яблока. На сегодняшний день сведения о влиянии витрэктомии на развитие струк-турно-функциональных изменений переднего от-резка глаза малочисленны и недостаточно инфор-мативны. Помимо анализа возможных изменений вследствие витрэктомии, авитреальные глаза можно рассматривать в качестве естественной модели для изучения структурно-функциональных нарушений переднего отрезка глазного яблока.Изменения клинической рефракции после витрэк-томии. Положительный исход витрэктомии описы-вают многие хирурги, однако в настоящее время ос-новное внимание обращено на функциональные результаты и качество изображения, в том числе рефракцию. В последние десятилетия рядом иссле-дователей выявлены данные, свидетельствующие об изменении рефракции после витрэктомии. K. Kumagai и соавт.[8] проводили ретроспективное иссле-дование пациентов (67 глаз) с различной витреоре-тинальной патологией, перенесших операцию уда-ления катаракты. Среднее значение рефракции че-рез 1 мес после вмешательства составило 0,3 дптр.
