Factors determining the duration of tracheal intubation in cardiac surgery: a single-centre sequential patient audit

Abstract: In an unselected sequential cohort, both patient- and surgery-specific factors may be influential in determining the duration of postoperative ventilation of the lungs following cardiac surgery. In view of the changing nature of the surgical population, regular re-evaluation is useful in reassessing performance.

“…Bispectral index monitoring did not affect the time to tracheal extubation of patients admitted to the ICU postoperatively. Time to tracheal extubation reflects illness severity rather than anaesthetic effect 19 , and times were longer in older patients with higher ASA scores having non-cardiac surgery. BIS monitoring during surgery therefore would not be expected to alter tracheal extubation time in ICU patients.…”

“…Although the impact on stays and costs is not universally reported [35,36], all studies show that early extubation in the first postoperative hours is well tolerated and does not increase respiratory complications. A recent Cochrane review [37 && ] evaluated the efficacy and safety of fast-track cardiac care in adults undergoing cardiac surgery, including early postsurgical extubation and the use of lowdose opiates in the general anesthesia.…”
