Background: Coronavirus disease highly contagious, is prevalent in all age and sex groups infecting the respiratory system. The present study seeks to investigate the epidemiology and effective factors in mortality of patients with COVID-19 in Ardabil province, northwestern of Iran.Methods: In a retrospective study, the hospitalized patients with laboratory diagnosed COVID-19 between February to August, 2020 were enrolled. Data registration portal was designated according to WHO recommendation. In this portal, demographic information, clinical presentation, laboratorial and imaging data were registered for patients in all hospitals in the same format. The Hosmer-Lemeshow guideline was used for variable selection in a multiple model.Results: Of the patients involved 2812(50.3%) were male and 150 (2.7%) had a contact with a confirmed case of COVID-19 in the last 14 days. Pre-existing morbidity was reported in 1310 (23.4%) patients. Of all patients 477(8.5%) died due to COVID 19. The hospitalization in ICU had significant correlation with fever or chills, shortness of breath, aches and pains, runny nose and chest pain.Conclusions: Hospitalization had the strongest effect on mortality followed by hospitalization in ICU, and higher age. This study showed that having some symptoms like aches and pains and headache can serve as preventive factors of mortality.

Este trabalho teve como objetivo analisar a associação entre confirmação ou não da Covid-19, comorbidades e casos graves da síndrome respiratória aguda - SRAG em Pernambuco-Nordeste-Brasil. Os dados foram coletados do Banco de Dados Open Data SUS do Ministério da Saúde, com informações do período 29/12/2019 e 14/09/2020. As análises foram feitas com o pacote rpart do software R, o qual constrói modelos de classificação ou regressão que podem ser representados como árvores binárias. Os resultados revelaram percentual significativo de cardiopatias e diabetes (média de 51% e 40%, respectivamente) presentes em pacientes com quadro de SRAG, independente da etiologia, nas três áreas consideradas. Observou-se, também, que pacientes cardiopatas desenvolvem SRAG com maior frequência quando acometidos com COVID-19 (64,6%) do que com outras etiologias (35,4%). O mesmo foi observado para pacientes diabéticos com COVID-19 (67,1%) e para pacientes negativos para COVID-19 (27,7%). A taxa de letalidade apresentou-se maior entre pacientes com as duas comorbidades, isoladas (47,6% para diabetes e 45,3% para cardíacos) ou em associação (50,3%), acometidos por SRAG causados por COVID-19. Os resultados obtidos são relevantes para elaboração de medidas de saúde pública visando identificar pacientes de maior risco e desenvolver tratamentos precoces e específicos para reduzir a mortalidade nesses casos.

Background: The current pandemic, COVID-19, caused by a novel coronavirus SARS-CoV-2 has claimed over a million lives worldwide in a year, warranting the need for more research into the wider determinants of COVID-19 outcomes to support evidence-based policies. Objective: This study aimed to investigate what factors determined the mortality and length of hospitalisation in individuals with COVID-19. Data Source: This is a systematic review with data from four electronic databases: Scopus, Google Scholar, CINAHL and Web of Science. Eligibility Criteria: Studies were included in this review if they explored determinants of COVID-19 mortality or length of hospitalisation, were written in the English Language, and had available full-text. Study appraisal and data synthesis: The authors assessed the quality of the included studies with the Newcastle Ottawa Scale and the Agency for Healthcare Research and Quality checklist, depending on their study design. Risk of bias in the included studies was assessed with risk of bias assessment tool for non-randomised studies. A narrative synthesis of the evidence was carried out. Results: The review included 22 studies from nine countries, with participants totalling 239,830. The included studies quality was moderate to high. The identified determinants were categorised into demographic, biological, socioeconomic and lifestyle risk factors, based on the Dahlgren and Whitehead determinant of health model. Increasing age (ORs 1.04-20.6, 95%CIs 1.01-22.68) was the common demographic determinant of COVID-19 mortality while living with diabetes (ORs 0.50-3.2, 95%CIs -0.2-0.74) was one of the most common biological determinants of COVID-19 length of hospitalisation. Review limitation: Meta-analysis was not conducted because of included studies heterogeneity. Conclusion: COVID-19 outcomes are predicted by multiple determinants, with increasing age and living with diabetes being the most common risk factors. Population-level policies that prioritise interventions for the elderly population and the people living with diabetes may help mitigate the outbreak's impact.
