RESUMENAntecedentes: Los Determinantes Sociales (DS) consideran "características del contexto social que afectan la salud, así como también los mecanismos por los cuales las condiciones sociales se traducen en impactos en salud". Las intervenciones cuyo objetivo son reducir la enfermedad serán exitosas solamente cuando consideren adecuadamente los DS de salud. Objetivo: Analizar, en base a la literatura existente, el modelo de determinantes sociales de la OMS y su aplicación a la adherencia al tamizaje de cáncer cérvi-couterino (CC). Resultados: Son variados los estudios que establecen la relación entre los determinantes estructurales e intermediarios y la adherencia al tamizaje. Si bien hay estudios contradictorios, es relevante la asociación positiva entre determinantes estructurales y mayor adherencia: un mayor nivel educacional, la presencia de empleo y un mayor ingreso familiar determinan una mayor adherencia al tamizaje. Como determinantes estructurales, son varios los factores reportados, entre los que se encuentran las características del hogar, el tiempo de permanencia, las características del sistema de salud, su disponibilidad y contacto con profesionales de la salud, factores de riesgo para CC, conocimiento acerca de la enfermedad así como también las creencias, entre otras. Conclusión: Es necesario considerar la prevención de CC desde una perspectiva mucho más amplia, con un enfoque que va mas allá de los fenómenos biomédicos que la falta de adherencia al tamizaje conlleva, sino como una problemática social que explica dicho comportamiento en salud. Se sugiere que las campañas de prevención de CC consideren el enfoque de DS.PALABRAS CLAVE: Cáncer del cuello uterino, Papanicolaou, determinantes sociales de la salud SUMMARY Background: Social Determinants consider "characteristics of the social context that affect health, as well as the mechanisms by which social conditions translate into health impacts". Interventions aimed at reducing diseases will be successful only when properly considered the social determinants of health. Objective: Analyze, based on the existing literature, WHO's Social Determinants of health model and its application to cervical cancer (CC) screening adherence. Results: There are many studies that establish the relationship between structural and intermediate determinants and adherence to screening. Even though there are contradictory studies, it is important the positive association between structural determinants and greater adherence: a higher education, employment and the presence of increased house holding come determine a greater adherence to screening. As structural determinants, several factors are reported, including house hold characteristics, residence time, characteristics of the health system, availability and contact with health professionals, risk factors for cervical cancer, knowledge and beliefs of the disease,

Context.— See, Test & Treat is a pathologist-driven program to provide cervical and breast cancer screening to underserved and underinsured patient populations. This program is largely funded by the CAP Foundation (College of American Pathologists, Northfield, Illinois) and is a collaborative effort among several medical specialties united to address gaps in the current health care system. Objective.— To provide an outline for administering a See, Test & Treat program, using an academic medical center as a model for providing care and collating the results of 5 years of data on the See, Test & Treat program's findings. Design.— Sources include data from patients seen at Tufts Medical Center (Boston, Massachusetts) who presented to the See, Test & Treat program and institutional data between 2010 and 2014 detailing the outline of how to organize and operationalize a volunteer cancer-screening program. Results.— During the 5-year course of the program, 203 women were provided free cervical and breast cancer screening. Of the 169 patients who obtained Papanicolaou screening, 36 (21.3%) had abnormal Papanicolaou tests. In addition, 16 of 130 patients (12.3%) who underwent mammography had abnormal findings. Conclusions.— In general, women from ethnic populations have barriers that prevent them from participating in cancer screening. However, the CAP Foundation's See, Test & Treat program is designed to reduce those barriers for these women by providing care that addresses cultural, financial, and practical issues. Although screening programs are helpful in identifying those who need further treatment, obtaining further treatment for these patients continues to be a challenge.

Cervical cancer is more common in the Somali immigrant population than the general population in the United States (US). There are low rates of cervical cancer screening among Somali women. This study compares cervical cancer screening test completion rates for a home human papilloma virus (HPV) test and standard clinic Pap test. Sixty-three Somali immigrant women aged 30–70 years who had not undergone cervical cancer screening within the past 3 years were randomly assigned to a home HPV test group (intervention) or a clinic Pap test group (control). Test completion rates were measured at 3 months. Univariate and multivariate logistic regression models were used to explore factors associated with test completion (intention-to-treat analysis). Participants in the HPV test group were 14 times more likely to complete the test compared to those in the Pap test group (P = 0.0002). Women who reported having friends/family members to talk about cancer screening were approximately three times more likely to complete any screening test than those who did not (P = 0.127) and participants who reported residing in the US longer were more likely to complete a screening test (P = 0.011). Future research should explore the potential of using the home-based HPV test kits as an initial approach to cervical cancer screening. Impact: The use of a self-sampling HPV kit has the potential to increase cervical cancer screening in under-served communities in the US.
