Factors Associated with Anatomic Failure and Hole Reopening after Macular Hole Surgery

Abstract: A macular hole (MH), particularly an idiopathic macular hole (IMH), is a common cause of central vision loss. Risk factors for nonidiopathic MH include high myopia, cystoid macular edema, inflammation, and trauma. MH is primarily diagnosed using slit-lamp microscopy and optical coherence tomography (OCT). Half of the patients with stage I MHs are treated conservatively and may show spontaneous resolution. The main treatment methods for MHs currently include vitrectomy and stripping of the internal limiting mem… Show more

“…OCT has strong tissue penetration, contains stereoscopic imaging, and provides more details of the lesions, thereby supporting the quantification of the lesions directly 7 . Based on OCT images, researchers have proposed a number of predictors of MH surgical efficacy 8 . However, most of these parameters are diameters of MH or intraretinal cysts (IRC), which cannot be used to fully describe the three-dimensional structure of MH.…”

“…The dynamic assessment of the repercussions of the surgical maneuvers on retinal tissue might lead to new insights into the biomechanical response of the retina to ILM peeling and, therefore, to better guiding the surgeon. New advancements in iOCT technology are needed and could contribute to settling controversies such as the need for a customizable size of ILM peeling, the technique of choice for each case (e.g., inverted ILM flap technique and, for that matter, the subtype: temporal, cabbage leaf, or any other) and the control of factors associated with anatomic failure after FTMH surgery [ 50 , 51 ].…”

“…There are various techniques described in literature including major surgical interventions (Reid et al, 2020) and minor procedures like gas tamponade injections for treatment of persistent MH (Iwase and 2007) (Kannan et al, 2016). Etiology for non-closure of MH is crucial in deciding the method of effective intervention (Li et al, 2021).…”
