Introduction: The segregation of people affected by leprosy in leper colonies as well as presenting itself ineffective, caused irreversible and irreparable consequences in the lives of individuals who came to carry the marks of stigma and prejudice surrounding the disease. Results:The stigma and the prejudice crystallized in our culture had caused and still cause huge suffering and pain in the participants, who grew up and several aged in biological terms, living with a stigmatizing disease and its complications, sometimes disabling ones. In social and psychological terms, they were discriminated, rejected and even expelled from the familiar conviviality and the original social nucleus. Conclusion:In this manner, one notices the need of these people recover their attachments, values and self-esteem, share feelings and build relationships so as to be integrated in the real world.

Abstract-Stigma is assigning a sign or label of a negative undesirable characteristic which is not appropriate for the society. In addition, stigma in the family is resulted from the loss of family support in patients with leprosy(leprosy sufferer), by excluding and hiding them until the cessation of the treatment process. This condition will aggravate the process of early detection of patients with leprosy, self-stigma in patients with leprosy, disability and quality of life of patients and family members. The aim of this study is to analyze the patient, family and environmental factors that influence the family stigma on leprosy. This study applied cross-sectional design while the study population was families of leprosy patients at six health centers for endemic leprosy in Jombang East Java and represent 49 families of leprosy patients. These samples included 39 families and sampling was with Multi-Stage Random Sampling. The hypotheses were tested by using Chi-Square with p value<0.05. The results found that there were 29 (74.4%) family stigmas occurred, while hypotheses tests obtained from the individual factors included stigma individual, self-concept and disability level of leprosy patient influence on family stigma (P = 0.000). Family factors include family support, family coping mechanisms, knowledge and family perception towards leprosy affecting the family stigma (P = 0.000), whereas there was no effect of economic status on family stigma (P = 0.07) and environmental factors affects the family stigma only as a social stigma (P = 0.007) and there is no stigma attached by healthcare providers. The conclusion is that the individual, family and environmental factors influence the occurrence of family stigma; therefore, the researchers recommend intervention in an attempt to overcome the family stigma attention on individuals, families and the environment.

Most scholarship on stigma begins with an explanation of Erving Goffman's Stigma: Notes on the Management of Spoiled Identity (Goffman 1963). However, since Goffman's now-classic work on the subject, there have been many linguistic shifts, conceptual slippages, and new contributions to the theorization and understanding of stigma. More recent interpretations of stigma emphasize the psychological and social aspects of stigma, as well as highlight the critical role of power and systemic inequalities which can work to produce or reinforce existing stigmas (Castro and Farmer 2005; Parker and Aggleton 2003). Link and Phelan (2001) stipulate that, in order for stigma to develop, there must be an element of power involved; they do not specifically state who it is that is exercising the power they mention, but surely the differential exists in favor of those doing the stigmatizing. Similarly, Castro and Farmer (2005) argue that for a more nuanced understanding of stigma and its effects in an infectious disease and treatment context, the power structures and inequities that produce poor access to care and treatment should be an integral component of the analysis of stigma production. Parker and Aggleton (2003:13) also argue that stigma "feeds upon, strengthens and reproduces existing inequalities of class, race, gender, and sexuality." There are a number of possible definitions of stigma. Parker and Aggleton (2003) suggest that many times, stemming from Goffman (though, they say, perhaps mistakenly so), authors treat stigma as an individual process and as a static, fixed item that exists. However, they argue that stigma also "plays a key role in producing and reproducing power relations and control…To properly understand issues of stigmatization and discrimination…requires us to think more broadly about how some individuals and groups come to be socially excluded, and about the forces that create and reinforce exclusion in different settings," advocating for analysis of stigma that takes into account its fluid and generative capabilities (Parker and Aggleton 2003:16).
