Chronic Kidney Disease is a disabling and high-cost chronic disease for patients who suffer from it and their families. When it manifests itself at a productive age and people are active at work, permanence in employment is at risk and the world of life of the patient’s changes in the face of physical damage, constant absences from the workplace, derived from medical consultations, studies and relapses; as well as secondary effects of renal replacement therapy. Faced with this situation, kidney patients of productive age who receive hemodialysis face multiple stressors in their day-to-day life to remain active in the workplace and economically independent. For this reason, they often resort to mechanisms and alternatives to deal with the disease and preserve their employment, autonomy, self-esteem, as well as control over them and the disease. The objective of this research is to describe, based on the experiences of a group of kidney patients of productive age who receive hemodialysis, the coping strategies they use to face the disease, recover their autonomy, self-esteem and control over them and the disease and continue to be active at work, either formally or on their own.

Objective: Immunobiogram (IMBG) is an in vitro diagnostic immune function bioassay that evaluates the pharmacodynamic immune response profile of each patient to individual immunosuppressants (IMS) in renal transplant. The objective was to estimate the potential economic and health impact of the use of IMBG for the Spanish National Health System (NHS). Methods: The evolution of a cohort of patients with kidney transplantation at least 1 year after transplantation (time horizon of 5 years) was simulated using a second-order Monte Carlo simulation for two scenarios: renal failure in patients with high immunological risk (HR), and adverse events (AE) in stable patients (non-HR). The transition probabilities were obtained from a clinical study with IMBG and a systematic review. The cost associated with graft failure (dialysis, re-transplantation), IMS and AE management were obtained from Spanish sources. Results: IMS adjustment, according to the IMBG could contribute to a risk reduction of graft failure with a saving per HR patient of €20,263 (95% CI €17,520-23,678) (100% saving probability). The expected reduction in the AE rate would generate savings per non-HR patient of €1,409 (95% CI €41-3,316) (97.8% saving probability). Compared with the option of not using IMBG, 0.5256 (95% CI 0.3388, 0.7452) years of life and 0.0219 (95% CI 0.0115; 0.0356) quality-adjusted life years (QALY) would be gained in each patient evaluated with IMBG. Conclusions: IMBG could contribute to a risk reduction of graft failure and AEs related with IMS, with gain in years of life and QALY, as well as with considerable savings for the NHS. Introducción: La enfermedad renal crónica es una patología progresiva e infradiagnosticada en sus primeros estadios y que va a necesitar un tratamiento renal sustitutivo en estadios avanzados, siendo el más utilizado la diálisis. Esto va a provocar un fuerte impacto en el bienestar psicosocial de los pacientes, con repercusión en su calidad de vida.Objetivo: Conocer la evidencia científica existente acerca de los factores psicosociales asociados a los pacientes sometidos a diálisis.Metodología: Se realizó una revisión bibliográfica sistematizada basada en la declaración PRISMA, en las bases de datos Pubmed, Proquest y Scielo. Para ello, se utilizaron una serie de estrategias de búsqueda en las que se utilizaron los siguientes términos Mesh: “Hemodyalisis”, “Peritoneal dyalisis”, “Factors psychosocial”, “Psychosocial cares”. La búsqueda se restringió a 5 años.Resultados: Se seleccionaron 28 artículos. Todos fueron de diseño observacional descriptivo, menos un ensayo controlado aleatorizado. De esta revisión emergieron como variables importantes: alteración del estado emocional, afrontamiento, situación laboral, depresión y ansiedad, apoyo social y calidad de vida relacionada con la salud.Conclusiones: Los principales aspectos psicosociales asociados a los pacientes en diálisis fueron la depresión y ansiedad, el apoyo social, la alteración del estado emocional, situación laboral y afrontamiento; siendo los más prevalentes la depresión y la ansiedad. La situación laboral y la depresión y ansiedad, se relacionan con una peor calidad de vida, mientras que la presencia de apoyo social y el estilo de afrontamiento de “evitación” mejoran la calidad de vida de estos pacientes.
