Extraadrenal Myelolipoma: A Report of Two Cases

Abstract: Two cases of extraadrenal myelolipoma with unusual clinicopathologic features are described. The patient in case 1 had multifocal myelolipomas, involving both the lung and retroperitoneum, that mimicked malignancy, and the patient in case 2 had a single diffuse retroperitoneal myelolipoma without circumscription. Of possible etiologic significance is the fact that both patients were treated with exogenous steroids.

“…They have been reported in the presacral region [2], mediastinum [3], retroperitoneum [4], liver [5], spleen [6], stomach [7], leptomeninges, and thoracic cavity including lung. To the best of our knowledge, only 8 cases of primary myelolipoma of the lung have been reported [8][9][10][11][12][13]. In the present paper, we report a case of pulmonary myelolipoma misdiagnosed as a malignant tumor before surgery.…”

“…The sites of extraadrenal myelolipoma have been described in the presacral soft tissue, retroperitoneum, pelvis, mesentery, spleen, liver, stomach, mediastinum, and leptomeninges [2][3][4][5][6][7]. Only eight cases of myelolipoma arising from the lung have been reported [8][9][10][11][12][13]. Of the previously reported cases, five of them were solitary, and two were multifocal.…”

“…The exact pathogenesis of myelolipoma is not clear at present. Most theories involve the development and differentiation of either ectopic adrenal rests or hematopoietic stem cell rests in response to a triggering stimulus, particularly endocrine dysfunction [15]. Other researchers found clonal cytogenetic abnormalities, which suggested it was of neoplastic pathogenesis [16].…”

“…They are predomi nantly composed of hematopoietic elements, erythroid predominance and lack of lymphoid aggregates. In con trast, myelolipoma is a well-circumscribed single mass and composed of adipose tissue and hematopoietic ele ments with lymphoid aggregates or follicle formation [41].…”
