La fièvre de la vallée du Rift (FVR) est une infection zoonotique émergente infectant les humains et les ruminants. L’agent étiologique de cette infection est le virus de la FVR, un arbovirus transmis principalement par les piqûres de moustiques des genres Aedes et Culex. Durant les deux dernières décennies, des cas de FVR ont été rapportés dans des régions jusque-là indemnes et les épidémies sont devenues de plus en plus fréquentes. De par sa localisation stratégique dans le bassin méditerranéen, la Tunisie a de nombreux échanges avec les autres pays africains ainsi qu’avec l’Europe. L’analyse de la situation épidémiologique de la FVR dans ce pays pourrait aider à comprendre la situation globale de cette infection dans la région méditerranéenne et à élaborer des stratégies efficaces de surveillance à mettre en œuvre dans la région. Dans ce cadre et afin d’étudier l’état des lieux de la FVR en Tunisie, cette synthèse présente une analyse à partir de tous les articles scientifiques traitant de ce sujet dans le pays. La Tunisie est considérée comme à haut risque de propagation de la FVR en raison de sa localisation, de ses caractéristiques climatiques et environnementales, de l’abondance des vecteurs transmettant le virus de la FVR, et de la présence d’espèces animales à risque pour cette maladie. Ainsi, des mesures strictes doivent être prises afin de contrôler et de limiter toute émergence et propagation de la FVR. De telles mesures doivent inclure le contrôle des mouvements des animaux (particulièrement aux frontières), le contrôle des espèces vectrices du virus, et le perfectionnement des outils de diagnostic pour la détection précoce de tout cas suspect. L’implémentation d’une approche « Une seule santé » multidisciplinaire serait l’une des meilleures solutions pour faire face aux maladies zoonotiques à transmission vectorielle comme la FVR.

Background Emerging infectious diseases pose an increasing threat to all nations around the world, including to developed countries. By definition, because they are rare or unknown, public health systems are not well prepared against these emerging diseases. To be fully prepared, countries must have implemented surveillance systems to monitor rare or unusual sanitary events. Methods The capacity of diagnostic laboratories is a key component of surveillance systems since they are in charge of identifying the pathogens responsible for outbreaks in a timely manner. The MediLabSecure project aims at implementing a comprehensive surveillance system for vector-borne diseases around the Mediterranean and Black Sea regions. From 2014 to 2018, the human-virology group of MediLabSecure notably supported the implementation of molecular diagnostic capacities for eight arboviruses and one coronavirus in 19 laboratories of its network through sharing of protocols and reagents, and technical training of the scientific staff of beneficiary laboratories. Results We report the results of External Quality Assessments for four of these viruses to assess the efficiency of the diagnostic for these threats emerging in the geographic area. The results for these EQA demonstrate the success of the project in the implementation of diagnostic technics for the identification of Dengue, Chikungunya, Zika, and West Niles viruses in laboratories that did not have the capacity before. However, results also show that some work is still to be done to strengthen the newly acquired capacity. Conclusion The MediLabSecure project deployed an effort to build an efficient capacity in identifying and survey the emergence of arboviruses in the Mediterranean area. Diagnostic technics were successfully implemented in many of the laboratories of the network, but the effort must be maintained over time to strengthen these capacities.

Emerging viral diseases pose significant threats to global health and economies, exemplified by recent epidemics of Ebola and Zika viruses. Rift Valley Fever (RVF), caused by the Rift Valley fever virus (RVFV), is a zoonotic disease primarily affecting domestic ruminants and camels. Originating i n Kenya in 1931, RVF has a broad geographic distribution, with endemicity in sub -Saharan Africa and occasional outbreaks in the Arabian Peninsula and beyond. The virus is transmitted through mosquitoes, with zoonotic transmission occurring through direct contact with infected animals or their products. Mosquito control, hygiene practices, and personal protective measures are crucial for mitigating zoonotic transmission risks. RVF outbreaks coincide with heavy rainfall and flooding, impacting livestock, trade, and human health. Risk factors include mosquito exposure, direct contact with infected animals, handling infected tissues, and consuming contaminated animal products. The pathogenesis involves entry, replication, systemic spread, immune response, hepati c involvement, hemorrhagic manifestations, neurological complications, and fetal complications in pregnant women. Clinical manifestations range from mild flu-like symptoms to severe hemorrhagic fever and neurological complications. Diagnosis involves serological and molecular tests, with vaccination, vector control, and surveillance essential for prevention and control. Future research priorities include vaccine development, enhanced surveillance, innovations in vector control, climate change impact studies, and antiviral therapies. Collaborative efforts, incorporating the One Health approach, are critical to addressing RVF challenges comprehensively. Continued investment in research and development is essential for advancing knowledge, improving prevention strategies, and minimizing the socioeconomic consequences of RVF outbreaks, safeguarding public health and animal populations.
