Extensively drug-resistant Klebsiella pneumoniae ST307 outbreak, north-eastern Germany, June to October 2019

Abstract: From June to October 2019, 17 patients (six infected, 11 colonised) with an extensively drug-resistant (XDR) Klebsiella pneumoniae strain were notified from four Western Pomerania medical facilities. The XDR K. pneumoniae produced carbapenemases NDM-1 and OXA-48, and was only susceptible to chloramphenicol, tigecycline and cefiderocol. Synergistic activity was observed for the combination of aztreonam plus ceftazidime-avibactam. Genomic analyses showed all isolates belonged to K. pneumoniae sequence type 307. … Show more

“…In this study, we analyzed carbapenemase-producing ST307 isolates, which have been recovered from screening and clinical samples within the course of an outbreak [19] that took place in four medical facilities in Western Pomerania, Germany, from June 2019 to October 2019. Additional cases were detected after the actual outbreak in the beginning of 2020.…”

“…Interestingly, and compared to other series of patients with NDM-producing K. pneumoniae [34][35][36][37], more than half of the adult patients were under 40 years of age and none were older than 55 years old, they were evenly divided between males and females, and four children were also affected. Notably, none of the isolates showed hypermucoviscosity upon performing the string test (data not shown), a trait that is often characteristic (albeit not specific) of hypervirulent strains [38].…”

“…MDR and XDR K. pneumoniae isolates involved in nosocomial outbreaks have been widely reported [ 4 , 21 , 22 ]. Between June and October 2109, an outbreak of XDR K. pneumoniae producing NDM-1 and OXA-48 was reported in four medical facilities in Mecklenburg-Western Pomerania, Germany [ 23 ]. Molecular characterisation using core genome multi-locus sequence typing (cgMLST) analysis revealed that the three investigated isolates were closely related and formed a cluster with 0–1 allelic differences (data not shown).…”
