IntroductionIn an ageing society, older adults are the main users of healthcare services, and Chinese healthcare systems have been struggling to meet the care demand of older adults. Due to the illness, many older inpatients cannot finish daily activities independently and require healthcare from caregivers. Evaluating the care needs of older adults and exploring the factors affecting safe care at multiple levels are conducive to providing systematic care services for older inpatients. This study aims to examine which and how factors impact safe care for older inpatients and propose a safe care theory to provide directions on improving the safe care system in the hospital.Methods and analysisA mixed-methods study with three interrelated research streams will be designed. (1) A safe care concept model: we will conduct a scoping review to extract the facilitators and barriers influencing the safe care of older inpatients and construct a concept model based on the socio-ecological model and the Yorkshire Contributory Factors Framework. (2) A safe care theory model: we will conduct a qualitative study with thematic analysis (aimed at older inpatients, caregivers and nurses) to define the concept connotations and propose a theory model. (3) A safe care theory: we will use the scales related to the theory model to test the concepts and statements. We will revise the theory model and derive a safe care theory based on the mixed-methods study results. This study will start in October 2023 and end in October 2025.Ethics and disseminationThis study was approved by the ethics committee (No. 20221598). All participants will be provided with consent forms prior to data collection. The study will be disseminated via peer-reviewed manuscripts and conferences.Trial registration numberChiCTR2300067421.

Eine Übersichtsarbeit aus Australien aus dem Jahr 2022 kommt zu dem Schluss, dass ausschließlich multimodale Interventionen mit entsprechender Ressourcen-Unterfütterung wirksam sind und dass keine wissenschaftlichen Daten vorliegen, die für die Effektivität von "Minimallösungen" sprechen [32].…”

ZusammenfassungTabakrauchen ist das größte vermeidbare Gesundheitsrisiko. Die Auswirkungen sind individuell und gesellschaftlich gravierend. Dennoch ist die Prävalenz aktuell Tabakrauchender in Deutschland mit ca. 35 % weiterhin hoch. Sorge bereitet zudem ein zuletzt starker Anstieg aktiv rauchender Jugendlicher (14- bis 17-Jährige, aktuelle Prävalenz ca. 16 %) und junger Erwachsener (18- bis 24-Jährige, aktuelle Prävalenz ca. 41 %). Etwa ein Drittel der stationär behandelten Patienten:innen rauchen. Die Hospitalisierung von aktiven Rauchern:innen in Akut- und Rehakliniken ist als „teachable moment“ ein günstiger Zeitpunkt, eine Tabakentwöhnung einzuleiten. Eine Intervention, die im Krankenhaus beginnt und nach der Entlassung mindestens einen Monat lang fortgesetzt wird, führt zu etwa 40 % zusätzlich entwöhnter Patienten:innen. Sie ist wissenschaftlich gut untersucht, effektiv und kosteneffizient. Die poststationäre Anbindung kann an ein Tabakentwöhnungsprogramm, eine Rehabilitationseinrichtung, ein Internet- oder Telefonangebot erfolgen. Es bestehen in Deutschland strukturierte und qualitätsgesicherte Angebote zur Umsetzung sowohl für den stationären als auch für den ambulanten Bereich. Größtes Hindernis für eine breite Etablierung solcher Angebote ist die fehlende Kostenerstattung. Zwei umsetzbare Wege, dies zu ändern, wären die Einführung eines Zusatzentgelts für den bestehenden OPS 9-501 „Multimodale stationäre Behandlung zur Tabakentwöhnung“ sowie die Etablierung von Qualitätsverträgen nach § 110a SGB V. Ein Ausbau der Tabakentwöhnung in Gesundheitseinrichtungen würde die Rauchprävalenz und die damit einhergehenden Erkrankungen sowie die konsekutiven Kosten nachhaltig reduzieren.

IntroductionChina has the largest youth population in the world. To better implement the Smoke-free School Initiative, this study aims to examine the protective and risk factors for different smoking behaviors (never smoked, experimental smoking, and current smoking) among school adolescents based on social cognitive theory.MethodsThis research was a secondary analysis of a cross-sectional survey of middle schools in Huli District of Xiamen, China. The final sample consisted of 1937 participants with an average age of 15.41 (SD = 1.64). Descriptive statistics were used to summarize the sociodemographic characteristics of the sample. Multivariate multinomial logistic regression analysis was performed using four models.ResultsOf the respondents, 1685 (86.99%) were never smokers, 210 (10.84%) were experimental smokers, and 42 (2.17%) were current smokers. Social norms, positive outcome expectations, anti-smoking self-efficacy, and attitudes toward control tobacco policies were associated with adolescents' smoking behaviors. The number of smoking family members, classmates smoking, the perception that smoking is cool and attractive, and attitudes toward control tobacco policies were the predictors of current smoking behavior (p < 0.05). In contrast, friends smoking and individual and social relationship motivation were associated with only experimental smoking (p < 0.05).DiscussionThe relationship of social norms, positive outcome expectations, anti-smoking self-efficacy, and attitudes toward control tobacco policies varied across smoking behaviors. Family, school, society and the government need to cooperate in prevention and intervention programs for adolescent smoking. The relationships between these factors and adolescents' different smoking behaviors needs to be further verified.
