Importance: In 2021, the American Academy of Pediatrics published a policy statement seeking to create a paradigm shift away from a focus on childhood toxic stress and toward the emphasis on early relational health (ERH) as a buffer for childhood adversity and promoter of life-course resilience. A comprehensive appraisal of the efficacy of contemporary parent/caregiver-child interventions in — primarily — improving ERH, and — secondarily — enhancing child wellbeing and neurodevelopment is needed to guide widespread implementation and policy. Objective: Determine the effectiveness of contemporary early dyadic parent/caregiver-infant interventions on ERH, child socio-emotional functioning and development, and parent/caregiver mental health. Data Sources: PubMed, Medline, Cinhal, ERIC, and PsycInfo were searched on April 28, 2022. Additional sources: clinical trial registries (clinicaltrials.gov, ISRCTN Registry, EU Clinical Trials Register, Australian New Zealand Clinical Trials Registry), contacting authors of unpublished/ongoing studies, backward/forward reference-searching. Study Selection: Studies targeting parent/caregiver-infant dyads and evaluating effectiveness of a dyadic intervention were eligible. Study selection was performed in duplicate, using Covidence. Data Extraction and Synthesis: Cochrane methodological guidance presented per PRISMA guidelines. Data extraction and risk of bias assessment were completed in duplicate with consensuses by first author. Data were pooled using inverse-variance random effects models. Main Outcomes and Measures: The primary outcome domain was ERH. Secondary outcome domains were child socio-emotional functioning and development, and parent/caregiver mental health, and were only considered in studies where at least one ERH outcome was also measured. The association between dose of intervention and effect estimates was explored. Results: 93 studies (14,993 parent/caregiver-infant dyads) met inclusion criteria. Based on very low to moderate quality of evidence, we found significant non-dose-dependent intervention effects on several measures of ERH, including bonding, parent/caregiver sensitivity, attachment, and dyadic interactions, and a significant effect on parent/caregiver anxiety, but no significant effects on other child outcomes. Conclusion: Current evidence does not support the notion that promoting ERH through early dyadic interventions ensures optimal child development, despite effectively promoting ERH outcomes. Given the lack of an association with dose of intervention, the field is ripe for novel, innovative, cost-effective, potent ERH intervention strategies that effectively and equitably improve meaningful long-term child outcomes.

Post-Natal Positioning through Babywearing: What the Orthopaedic Surgeon Needs to Know

Babywearing is the practice of using a swathe of fabric or purpose-built carrier to hold an infant or toddler close the caregiver’s torso, for multiple hours of the day. The child’s legs are often positioned in an “M” shape, with hips and knees flexed and abducted. There is increasing interest in the potential for babywearing to assist in hip development, as the “M” position assumed in most carriers is similar to the position achieved in harnesses/braces used in the treatment of developmental dysplasia of the hip (DDH). The association between low incidence of DDH in babywearing populations (Southern China, Aboriginal people of Western Australia, Central and Southern Africa, Malawi, and the Inuit/Eskimos of the circumpolar north) suggests that babywearing may play a role in optimal hip development, although genetics likely also contributes. Biomechanical studies have demonstrated optimal hip joint reaction forces while in baby carriers, with similar lower-extremity muscle activation as obtained in Pavlik harnesses. Ultrasound studies have shown that infant hips maintain normal ultrasound parameters while in baby carriers. In addition to its potential positive impact on hip development, babywearing stimulates caretaker-infant responsiveness, improves attachment and bonding, promotes language development, decreases infant crying, supports breastfeeding, and allows for caregiver multi-tasking. With improper use or insufficient care, asphyxia, falls/trauma, and baby-carrier purpura are potential complications. Energy expenditure of babywearing is less than arm-carrying, and effects on gait and posture can be minimized with proper positioning. Orthopedic surgeons may support their patient families that wish to babywear, as there are multiple non-orthopedic benefits, few potential complications if performed properly, and a promising potential for this form of post-natal positioning to have a positive impact on hip development. Future research should focus on prospective comparative studies of babywearing versus other post-natal positions and their impact on hip development.

Aims: This study aims to study, analyze and reveal the factors influencing the interest in purchasing Soft Structured Carriers (SSC) in millennial mothers in the Jabodetabek area. Study Design: Cross Sectional Quantitative Research. Place and Duration of Study: 271 millennial parents in Jabodetabek who have bought soft structured carrier (SSC) at least once and still have kids below 1-year-old. Methodology: This study used a quantitative with causal research approach and included 271 respondents (24 living in Depok, 32 living in Bogor, 56 living in Bekasi, 61 living in Tangerang, and 98 living in Jakarta; age range 25-40 years). The data gathered using questionnaire. Sample methods using the guideline from Hair et al. (2017); therefore, in this study, based on purposive (judgemental) sampling techniques and being analysed using Partial Least Square Structural Equation Model (SEM). Results: The results of this study found that the direct relationship between risk communication (RC) had a positive and significant effect on perceived risk (PR) and purchase intention (PI), PR had a positive and significant effect on PI, but for subjective norms (SN), it was found that it did not affect PI. RC to PR has the most significant influence compared to other relationships. Attention to the RC factor can increase buying interest by only causing a PR factor in people's minds. Furthermore, in the indirect relationship, it is found that PR mediates the relationship between RC and PI. Therefore, PR has effectiveness in connecting RC with purchase intention. Conclusion: Of the five hypotheses tested, one of them has no influence where Subjective norms were found not to affect Purchase intention. In indirect hypothesis testing, it is found that Perceived risk mediates the relationship between risk communication and purchase intention.
