Relevance. A viral pandemic caused by the SARS-CoV-2 coronavirus has led to the development of a new coronavirus disease-2019 (COVID-19). The COVID-19 pandemic has forced the mobilization of all available health system resources. There are separate publications on reducing the risk of developing coronavirus infection in people vaccinated against influenza. Objective: to study the cost-effectiveness of influenza vaccination in the conditions of the» first « wave of COVID-19. Materials and methods. The archival data of 2,452 people from among the sick employees of JSC «Russian Railways» were analyzed. The control group consisted of 2,911 employees who were not infected with COVID-19, comparable by gender, age and territory of residence. Scores on the Charlson comorbidity scale were calculated for all individuals. The pharmacoeconomical cost of the patient’s treatment was predicted using the Markov model. Results. Having a flu shot reduced the likelihood of getting COVID-19 by 1.3 times. In the presence of a diagnosis of coronavirus infection, inpatient treatment for influenza vaccinated patients was required 2 times less often than for unvaccinated patients. Compared to the situation of the absence of vaccinated persons, in the «first wave», the estimated cost savings for the treatment of patients with coronavirus infection amounted to 124 million rubles. When the number of points on the comorbidity scale increased from 1 to 8, the average cost of treatment of patients without previous influenza vaccination increased by 2 times, and in the presence of vaccination, the average cost of treatment increased by 1.7 times. Conclusion. Thus, this study shows that influenza vaccination is cost-effective against COVID-19. The effect is achieved by reducing the likelihood of getting a coronavirus infection in the presence of a flu shot.

Features of the incidence and course of COVID-19 among medical workers of private healthcare organizations of JSC "Russian Railways"

Healthcare workers are at risk of contracting new coronavirus infections (COVID-19) due to their job responsibilities. The study aims to explore the incidence of a new coronavirus infection of medical workers of private healthcare organization (PHO) of JSC "Russian Railways". To analyze the incidence of COVID-19 among medical workers of PHO of JSC "Russian Railways", we used the data of weekly monitoring conducted by the company from 5.04.2020 to 28.03.2021. The χ2 method was used for statistical processing. The epidemiological data were also smoothed by linear filtering using 2 points. A feature of the medical service of PHO of JSC "Russian Railways" is the continuous nature of its activities. 5 hospitals were converted to COVID hospitals. 12.9% of cases of COVID-19 were recorded among employees of PHO of JSC Russian Railways. The minimum number of cases was registered on the Far Eastern and East Siberian Railways. The maximum number of cases was noted on the South-Eastern and Northern railways. 94% of the sick were on outpatient treatment. Half of the sick are nurses. The testing of medical workers of PHO of JSC "Russian Railways" revealed a higher percentage of people with IgG to the SARS-CoV-2 virus than the number of cases registered. This may indicate an asymptomatic course of the disease in medical workers. Conclusion. The data obtained indicate the effectiveness of preventive measures to protect medical workers of PHO of JSC "Russian Railways" from a new coronavirus infection. The conducted research will increase the effectiveness of preventive measures in the other PHO.

Railway transport performs an essential economic and social role. Moreover, railway transport is steadily developing in terms of engineering technologies, strategically and economically, ensuring the development of remote and hard-to-reach regions of the Russian Federation. Factors that negatively affect the health of railway transport workers, primarily locomotive crews, researchers divided into several groups: physical production factors (noise, vibration, electromagnetic influences, workplace microclimate); factors related to the peculiarities of the production process organization (shift work schedule, irregular working hours, processing); behavioral risk factors (compliance by employees of locomotive crews with the principles of rational nutrition, recommendations for physical activity, the presence or absence of bad habits). Combining these risk factors affects railway transport workers and can lead to the potentiation of their combined effect. Each group of persons involved in railway traffic has its risk factors and their combinations in one way or another. The review provides a detailed description of the principal risk factors affecting the health of railway transport workers.
