Évolution du profil sociocognitif et clinique d’adolescents diabétiques de type 1 ayant suivi un programme d’éducation thérapeutique : une étude pilote

Abstract

Introduction : L'éducation thérapeutique est une des clés de la réussite d'une prise en charge de la maladie diabétique pédiatrique. De nouveaux programmes ont été mis en oeuvre en France. Objectif : Décrire l'évolution du profil sociocognitif et clinique sur trois mois d'un groupe d'adolescents diabétiques de type 1 ayant suivi un programme d'éducation thérapeutique. Méthode : Cette étude pilote observationnelle prospective s'appuie sur une méthode de recherche mixte en trois volets : quantitatif, qualitatif et intégratif. Une population d'adolescents diabétiques de type 1, âgés de 12 à 17 ans, a été recrutée pour participer au programme d'ETP et à l'étude durant 3 mois. Résultats : Vingt-quatre adolescents ont participé au programme d'ETP. Dix-sept d'entre eux ont complété le volet quantitatif sur trois mois, et 12 ont finalisé le volet qualitatif. L'évolution du sentiment d'efficacité personnelle et l'évolution du taux d'HbA1c avaient tendance à être inversement corrélés (r = -0,474 ; p = 0,075). Un profil sociocognitif a permis d'identifier les forces et difficultés de ces adolescents dans la gestion des traitements et de la maladie. Le discours des adolescents présentant une augmentation du sentiment d'efficacité personnelle était orienté sur les compétences d'autosoin et psychosociales, tandis que celui des adolescents présentant une baisse du sentiment d'efficacité personnelle montrait de la lassitude et des préoccupations en lien avec la maladie et leur santé. Discussion : Des recherches complémentaires sont nécessaires pour confirmer les résultats de cette étude pilote. Conclusion : L'implication systématique des adolescents, des proches et des équipes de diabétologie pédiatrique devrait permettre de proposer des ateliers correspondant au mieux à leurs besoins et à leurs attentes en fonction du contexte de déroulement du programme.
