Background The evolution of retention forces in one‐piece mini dental implants (MDIs) retaining implant overdentures (IODs) is of major importance, as the male parts cannot be exchanged, due to the implant design. Purpose To report the evolution of retention forces of one‐piece MDIs, retaining mandibular IODs with ball/ O‐ring attachments during 5 years assessed in vivo. Materials and Methods Four MDIs were installed in the interforaminal region and immediately loaded using the existing mandibular complete denture. Directly post‐op, at the 1‐year and the 5‐year follow‐up, the retention forces were assessed with a validated strain gauge at each implant site separately. Results At the male part, changes could only be observed at implant site 34: During the first year, there was a statistically significant increase, whereas the retention forces were decreased at the 5‐year follow‐up. At the female part, retention forces decreased significantly over time. Baseline values could be reestablished by exchanging the O‐rings. The changes of the retention forces were more obvious in the posterior, compared to the anterior implants. Conclusions Retention forces at the female part decrease significantly over time, when retaining mandibular IODs by MDIs with ball/ O‐ring attachments. Baseline values can be reestablished by exchanging the O‐rings. At the male part, changes of retention forces depend on the implant location.

BackgroundThis study was aimed to evaluate the influence of inter-implant distance on the wear characteristics of low-profile stud attachments used in mandibular implant retained overdentures.Material and MethodsForty Completely edentulous participants aged between 50 – 70 years were enrolled in this study. Each patient received 2-implants by 2-stage submerged surgical protocol. Participants categorized into 4-groups. Group I: 19 mm inter-implant distance with Locator retained overdentures; Group II: 19 mm inter-implant distance with OT Equator retained overdentures; Group III: 25 mm inter-implant distance with Locator retained overdentures; Group IV: 25 mm inter-implant distance with OT Equator retained overdentures. The female housings of each attachment were picked up to the mandibular overdenture. 12 month later the male inserts were replaced by new one. The used retentive male inserts were examined by Stereomicroscopic.ResultsStereomicroscopic examination revealed wear were detected on both inner surface and, the core of male inserts. Comparison between the unused and the used Locator and OT equator retentive male inserts at various inter-implant distance revealed highly significant wear changes between them at either 19 mm, or 25 mm inter-implant distance P1= .000, P2=.000 respectively.ConclusionsAfter one year of implant overdenture clinical use; both locator and OT equator retentive male inserts revealed significant surface deformities and wear. Wear were more notable on both locator and OT equator retentive male inserts with 25 mm interimplant distance than with 19 mm interimplant distance. Retention forces between primary and secondary CAD/CAM manufactured telescopic crowns: an in vitro comparison of common material combinations

Objectives To analyze the retention forces between primary and secondary telescopic crowns milled from various materials and to compare them with the retention forces between cast telescopic crowns made of precious metal alloys. Materials and methods Primary and secondary crowns (N = 60; n = 10 per group) were fabricated using various material combinations (1: zirconia [ZIR]/polyether ether ketone [PEEK]; 2: titanium grade IV [TI]/PEEK; 3: PEEK/PEEK; 4: non-precious alloy [NPA]/PEEK; 5:NPA/NPA), while precious alloy (PA) was used for the control group (6: PA/PA). The retention forces at 10, 1000, 5000, and 10,000 connection and disconnection cycles and the relative weights were analyzed, applying nonparametric repeated measures ANOVA and post hoc Mann–Whitney and Wilcoxon signed-rank tests (α < 0.05). Results Globally, significant differences in the retention forces among the materials (p < 0.0001), time points (p < 0.0001), and wear resistance for the various materials (p < 0.0001) were observed. No significant changes in retention forces compared to baseline were observed in groups 2, 4, 5, and 6. A significantly higher weight loss for both primary and secondary crowns was observed in groups 4 and 6. Conclusions The material combination in telescopic attachments influences retention forces and wear. Interactions between materials and time were evident, indicating that the change in retention forces differs among the materials. The combinations of milled TI/PEEK and NPA/NPA qualify for further preclinical testing in a more clinically realistic setup, determining a material-specific double-crown design. Clinical relevance The design of precious alloy telescopic crowns cannot be directly transferred to other milled material combinations due to different retention behaviors.
