Background 1,4-butanediol diglycidyl ether (BDDE) is the most common cross-linker used to produce hyaluronic acid (HA)-based dermal fillers. However, BDDE may have cytotoxic and potentially mutagenic effects, raising safety concerns. Consequently, manufacturers are developing new HA filler formulations with reduced BDDE levels to mitigate potential biological risks. Here, we sought to evaluate the clinical outcomes of lip augmentation performed using an HA-based filler with a reduced BDDE content (Agex Fill Volume ® ; Biodue SpA, Barberino Tavarnelle, Italy) in a real-world clinical setting. Methods This was a prospective, open-label, multicenter, post-marketing study conducted over six months. Thirty adult subjects (29 women and one man) who desired a ≥1-point improvement on the validated Lip Fullness Scale 2 (LFS2) were enrolled. The primary efficacy endpoint was the post-procedural increase in the investigator-reported LFS2 compared to baseline. Other endpoints included self-perceived happiness assessed using the Happiness Measure Scale (HMS) and safety. Results Of the study participants, 73% (22/30) demonstrated an improvement of at least one point in their LFS2 scores immediately after treatment compared to baseline, thus qualifying as responders. Six months later, the responder rate, based on LFS2 scores, remained steady at 66.7% (20/30). Importantly, these aesthetic improvements were consistently associated with a positive impact on subject-reported HMS, with a significant difference (p < 0.001) between post-treatment and baseline scores. All adverse events (AEs) reported after treatment were mild. Conclusions Agex Fill Volume ® , a HA filler with low BDDE content, provides a safe and effective option for enhancing lip volume in real-world aesthetic settings.

Over the preceding five decades, there has been a noticeable surge in the pursuit of achieving voluminous and well-defined lips. This trend has prompted an escalating number of individuals to undergo lip augmentation procedures, aiming for a natural three-dimensional enhancement of lip volume and distinct vermilion borders. Despite the proliferation of lip augmentation techniques, there remains a dearth of comprehensive investigations into their precise effects on the three-dimensional structural integrity of the lips. This research endeavors to address this gap by employing stereophotogrammetry as a quantitative tool to scrutinize lip augmentation outcomes and to appraise the efficacy of lip plumpers. The study methodology involves a comparative analysis of lip dimensions among subjects treated with a commercial lip plumper using multi-spectral imaging for lip dimension assessment, coupled with markerless tracking technology and 3D interpolating surface methodology to analyze lip volume and shape. Additionally, the study evaluated lip youth state, including moisture level, softness, firmness, and tissue density. The demand for lip augmentation procedures is driven by perceived advantages such as quick recovery and minimal risk. Therefore, it is crucial to substantiate their efficacy with robust findings. The investigation suggests that both 3D and 2D stereophotogrammetry techniques are reliable for evaluating lip size before and after augmentation, whether through cosmetic or aesthetic approaches. Overall, the study provides a comprehensive analysis of a lip treatment aimed at enhancing volume and redesigning lip lines. It demonstrates that stereophotogrammetry is effective for assessing 3D lip dimensions and their correlation with internal lip structure. This research could be particularly valuable for evaluating the efficacy and duration of various lip enhancement techniques, including dermal fillers, implants, and topical cosmetic formulations, offering quantitative and reproducible assessments over time.

Introduction The vermillion lip has unique physical properties and environmental exposures make them prone to dryness and chapping. While lips contribute to facial beauty, perioral aging is highly individual and dependent on ethnicity. Specifically with regard to South Asian population, there is no other literature identified on the biophysical characteristics of the vermillion lip. Methods In this work, characteristics of the vermillion skin in South Asians were examined through various clinical and biophysical characteristics of the lips such as dryness and roughness, fine lip lines, lip texture, hydration, and barrier function. Healthy South Asian females, from age 20–45 were recruited for this study and transepidermal water loss (TEWL), hydration, visual lip tolerance, and visual dryness were measured and graded by the dermatologist-investigator. Results Lip texture and fine lines significantly increased from the 20s to the 40s. Overall lip health significantly decreased with age. TEWL and dryness increased with age while hydration decreased with age, although not significantly so. Discussion This first-of-its kind study on South Asian females establishes that with age there is a decrease in overall lip health and an increase in signs of aging of the lips (fine lines, texture) from the 20s to 40s.
