Background: The main feature of diabetes mellitus (DM), a complicated metabolic illness with significant global impact, is persistent hyperglycemia brought on by either insulin resistance or deficiency, or both. Aim: To assess platelet indices in type 2 diabetic patients with or without complications. Study Design: cross-sectional study. Methodology: This study was carried at Combine Military Hospital-Multan. Total patients (n=120) were enrolled and were divided into two equal groups with complication and without complication. In addition to providing standard follow-up care, the diabetes follow-up clinic screens all DM patients for diabetes-related complications at the initial visit. Patients were then checked for these issues every six months. The data was entered and analyzed in SPSS 23. Mean ± SD were given for numeric data i.e., age and platelet indices. The frequency and percent were calculated for categorical data i.e., gender. Independent sample t-test was used to compare the age and platelet indices between the two groups. Chi-square test was used to compare the gender distribution between the two groups. A p-value ≤ 0.05 was considered significant. Results: Independent sample t test revealed that there was no significant difference in mean age between both groups. The results revealed that the mean PDW, MPV, PLCR and PCT levels was significantly higher in patients with complicated diabetes. Practical implication: This study helped in exploring platelets indices as platelet indices results have notable impact on patient diagnosis and follow-up. With increasing demands and burden of patients in Hospital, new reliable and quick diagnosis is need of hour. Conclusion: It was concluded that all platelet indices like MPV, PDW, and P-LCR increased significantly in patients with complicated diabetes. However, platelet indices can be used as a simple, low-cost metric to track and gauge the likelihood of problems. Purpose Platelet indices have been shown to be associated with complications of diabetes mellitus (DM). However, the clinical relevance of platelet indices for assessing the risk of being microvascular complications among patients with DM is largely overlooked. Hence this study aimed to determine the association of platelet indices with microvascular complications among type 2 DM patients. Patients and Methods A hospital-based cross-sectional study was conducted among 352 type 2 DM patients from April 1 to May 30/2019 at Debre Berhan Referral Hospital. Data were collected by interview, physical measurements, document review, and laboratory tests. Then the data were entered into Epi-data manager-4.4.1.0 and exported into SPSS-25 for analysis. Platelet indices were compared between participants with and without microvascular complications using an independent t -test. A logistic regression model was used to assess the association between microvascular complications and platelet indices. All inferences were considered statistically significant at P<0.05. Results One hundred seventeen (33.2%) of participants were suffering from at least one microvascular complications. The mean platelet volume (MPV), platelet distribution width (PDW), and platelet-large cell ratio (P-LCR) were significantly increased in DM patients with complications as compared to without complications (13.57±2.17fl vs. 11.76±1.93fl), (16.57±2.49fl vs. 14.97±2.41fl) and (28.09±7.58% vs. 24.19±6.46%), respectively (all are P˂0.001). In multivariate logistic regression analysis, MPV (AOR=1.68, 95% CI: 1.37–2.05), PDW (AOR=1.37, 95% CI: 1.15–1.63), P-LCR (AOR=1.07, 95% CI: 1.01–1.14), age (AOR=1.07, 95% CI: 1.01–1.12), and duration of DM (AOR=1.31, 95% CI: 1.18–1.46) were significantly associated with the presence of microvascular complications. Conclusion MPV, PDW, and P-LCR were significantly higher among type 2 DM patients with microvascular complications. This indicates that platelet indices can be used as prognostic markers of vascular complications and can be used as simple and cost-effective parameters to predict microvascular complications particularly in patients with old age and long duration of DM.

Diabetes mellitus leads to long-term complications involving multiple organs and systems of the human body. Among the list of complications, a relatively vascular complication increases the morbidity of the condition. Patients with diabetes mellitus are influenced by various factors like hyperglycaemic state, insulin resistance, oxidative stress, and metabolic condition like obesity, and all the mentioned conditions also present with platelet hyperactivity. Mean platelet volume (MPV) can be used as one of the laboratory parameters to know the function and activation of the platelets, which reflects the vascular profile of the patient. So, the present study compares the values of mean platelet volume among the diabetic groups to determine the relation between the vascular complication and the mean platelet volume. This study was conducted with 90 participants, who were divided into three groups. Group A is non-diabetics, group B is type 2 Diabetics, and Group C is type 2 Diabetics with vascular complications and MPV. On analyzing the statistical mean value of mean platelet volume, group B's (type 2 diabetics) value was higher than group A's (non-diabetics) and statistically significant with a p – value of 0.001. Similarly, the mean value of group C (type 2 diabetes with complications) was higher than group B (type 2 diabetics) and statistically significant with a p – value of 0.049 in the diabetics with and without vascular complications. On comparing the MPV of different study groups, the MPV is higher in the diabetic group with complicatiosn compared with the diabetics without complications.
